  2. Do I Need To Re-enroll? • Health Care Flexible Spending Account or Dependent Care Spending Account. Yes, if you want to participate in an FSA. • Health, Dental & Vision plans. Yes, if you would like to make any changes to the coverage level of your existing medical plans (health, dental and vision coverage), or if you would like to change health or dental plans or if you wish to drop coverage. • Life and AD&D coverage. Yes, if you would like to make any changes to the coverage level of your Life Insurance or AD&D coverage and/or to designate new beneficiary(ies).

  3. Important Changes & Updates Health Plan • Out-of-Pocket Maximum will include all deductibles, copayments and coinsurance and be assigned to all health plans to comply with Health Care Reform. • Music therapy will be included in the covered therapies. • The combined therapy maximum will be increased to 45 visits on the UK-PPO and UK-EPO plans to match the UK-HMO and UK-RHP. • Copayment decrease for therapy visits under UK-PPO and UK EPO. • UK-HMO and UK-RHP copayment increases high end diagnostic and hospital observation by $25 per visit.

Important Changes & Updates Health Plan Premiums • Health plan premiums will increase 2% overall and employees who receive the health credit will see a $1-$7 increase per month for UK-HMO, UK-PPO and UK-RHP plans. UK EPO premiums will increase between $5-$14 per month. • The overall health plan costs are increasing by only 2%. The university is contributing an additional $2.1M to cover the increased cost for FY 2014-15.

Important Changes & Updates UK-RHP • Provider Regions have been expanded, increasing your options when choosing a doctor. • If you reside in Region A, you can now see any provider located in Region A. (see map) • If you reside in Region B, you can now see any provider located in Region B. (see map)

  7. Important Changes & Updates Dental Plans • Delta Dental premiums will remain unchanged and UK Dental premiums will increase between 1 and 10%. Vision Plan premiums will remain unchanged. Life and AD&D Plans • Dependent Life coverage option for $5,000 will be eliminated and $20,000 option will be added. • AD&D maximum will increase to $500,000. • No increase in optional life premiums and decrease in AD&D premiums.

  8. Important Changes & Updates Health Care Flexible Spending Accounts • New! You may now roll over up to $500 from your Health Care FSA left unused to the following plan year. • You will still have the grace period (9/15/2014) to incur claims for FY 2013-14 and you can file claims through 12/31/14. The grace period is ending beginning with the 2014-15 plan year and being replaced with the $500 Rollover. You will be able to rollover up to $500 on June 30, 2015.

  9. ID Cards Members will receive new ID cards if currently enrolled in: • Any health plan • Anthem Blue Cross and Blue Shield Members will receive ID cards ONLY if newly enrolled in: • Delta Dental • Express Scripts • EyeMed Vision • ConnectYourCare *UK Dental does not issue ID cards

Transitioning Rewards Opportunity from Healthtrac to a New & Improved Method Over the past 11 years, Healthtrac Rewards has engaged our population by making them aware of their health status and providing financial rewards for participating in healthy programs/behaviors to improve their health. Significant risk reduction over the past 3 years: • Cholesterol, Blood Pressure, Smoking, Average # of health risks, increase in preventive exam utilization • ROI = $1.3 M from 2006-2009 for Healthtrac participants. New rewards opportunity will be available FY 14/15.

The incentives in this new & improved rewards opportunity will be tied to: • “Know your numbers” campaign – health screenings (utilizing real data vs. self reported data) • Increasing physical activity • And more! This rewards opportunity will be: • Easier to participate in, with an intuitive registration process • More relevant and meaningful to the individual through more targeted communications and interventions

Life and AD&D Insurance • Basic life insurance is 1x annual salary • University paid premiums for coverage above $50K are taxable. • Optional life insurance available up to 5x salary • Coverage above $375k is subject to medical evidence of insurability. • Increases of more than one level optional life coverage are subject to medical evidence of insurability. • Dependent life coverage available (children up to age 26) may purchase up to $20,000

  23. Flexible Spending Account Programs Save You Money! Health Care Spending Account Set aside between $250 and $2,500 per year before taxes for medical, dental, and vision expenses not covered by a medical plan (excluding premiums). Dependent Care Spending Account Set aside between $500 and $5,000 per year before taxes for day care, pre-school, and after school programs to enable you and your spouse to attend work and/or school.

Flexible Spending Account • Administered by ConnectYourCare. • UK requires all participants to use direct deposit. • Roll over up to $500 of unused funds from the 2014-15 plan year to the following plan year. • Use left over money from 2013-2014 plan year until September 15, 2014. • Claims may be filed with ConnectYourCare using a claim form. • File claims for 2013-14 by 12/31/14.

Flexible Spending Debit Card • Prepaid Card • Cards will be mailed to you automatically from ConnectYourCare if you are a new participant. • Quick access to health care flexible spending funds. • Present the card as a credit card for doctor office copayments, prescription and other eligible expenses. • Keep your receipts –some purchases require follow-up (KY Clinic Pharmacy, some medical and dental). If you purchase prescriptions using your Express Scripts coverage, the expenses will be automatically processed.

Dependent Documentation(Open Enrollment and New Hires) • To enroll any dependents who currently are not covered, or who have not been covered and documented in the past on a benefit plan, you will be asked to provide documentation to prove the relationship. • Spouse: marriage certificate and front page of tax return • Adult Sponsored Dependent: documentation of one-year common residency required (see www.uky.edu/HR/benefits/SposoredDependents for documentation requirements) • Child: birth certificate; adoption; guardianship; court order, or front page of the tax return, etc.

  27. Employee Education Plan Employee Education Program (EEP)- employees may have cost of up to 6 credit hours per semester waived, up to 18 per academic year max. Participation Fall 2013 (1618) & Spring 2014 (1511) Family Education Program (FEP)- employees who do not use EEP may elect FEP tuition discount for spouse, sponsored dependent or dependents. Participation Fall 2013 (593) & Spring 2014 (518) • Employee must have at least one year of FT consecutive service; benefit ranges from 10%-50% discount • University of Kentucky undergraduate classes only

Staff Shared Leave Pool • Staff shared leave eligibility (0.5 FTE or greater) • Benefits employees who exhaust accrued paid leave to care for a spouse, sponsored-dependent or child. • Staff donation period: May 1st – June 30th (September 30Healthcare ) • Donate by going to HR Employee Relations Website: http://www.uky.edu/hr/employee-relations/shared-leave-pool/about-staff-shared-leave-pool

Financial Counselor • Financial Counseling is a free voluntary counseling and referral service for UK employees (regular, .5 FTE or greater). • Intended to help employees gain control of their finances. • Up to four sessions are available per fiscal year. • Sessions generally last approximately 1 hour. • Appointments may be scheduled Monday through Friday by calling Mike Pelfrey at 859-257-9487 or visiting http://www.uky.edu/hr/benefits/retirement-financial-security/financial-counseling

CRISIS Program • This program provides temporary financial assistance in the form of a one-time payment of up to five hundred dollars to University of Kentucky staff members who are experiencing personal hardship. • Donate online through ESS. All donations are non-tax-deductible and payroll deductions are post-tax. • Administered by UK Staff Senate

Benefits Open Enrollment:Enroll Online! • Visit the myUK website at https://myuk.uky.edu  Enter the link blue user ID and password. • Select “Employee Self-Service.” • Click on “Benefits and Payment” and then “Open Enrollment” under “Adjustment Reasons.” • Click on “Review Enrollment” when you are through with your selections and then “Save.” • PRINT- click on “Print New Selections” and save a copy for your records.

Key Points • Benefits Open Enrollment Dates - Monday, April 28th through Friday, May 16th. *Drop box available for forms outside Scovell Hall side entrance. • The UK health plans will have a small premium increase. • There will be a new $500 rollover to the Medical Flexible Spending Account in place of the 2 ½ month grace period to incur claims. • Following ACA requirements, health plan out-of-pocket maximums will include all deductibles, copayments, and coinsurance excluding prescription coinsurance. • Dependent Life coverage will have $20,000 option available and the $5,000 option will be discontinued. (Minimum coverage of $10,000.)
