Centralized CAF Process for Supporting Children with Additional Needs

The Common Assessment Framework (CAF) process centralizes the collection of all CAFs. Submissions should be made to Claire Flexney at Integrated Services, Meadow House, Mansfield, or via password-protected email to caf@nottscc.gov.uk. Ensure to identify any additional needs of the child under the Every Child Matters Outcomes. Gain written consent from families when discussing the child's situation. Engage with a multi-agency team to create a comprehensive action plan, address safeguarding concerns, and conduct regular review meetings to update the CAF as necessary.

The CAF Process All CAFs are collected centrally, please send recorded delivery to; Claire Flexney Integrated Services, Meadow House, Littleworth, Mansfield, NG18 2TB Or password protected to; caf@nottscc.gov.uk Has the child got additional needs under the 5 Every Child Matters Outcomes? If not sure what to do (with consent from the family, or if there is no consent the child can be discussed anonymously) Discuss with child/young person and family Gain written consent for CAF Log CAF Information Sharing Project Team: 0115 9772801 Complete CAF form as far as possible If need further advice & support At each stage, consider if there are any safeguarding concerns If so, undertake the appropriate action & discontinue the CAF process Child or young person’s needs met by assessing agency Joint Access Team Multi-agency / Team Around the Child Meeting - Agree Multi-Agency Action Plan - Lead Professional agreed - Date of Review Meeting Set - Update Information Sharing Project Team Continue CAF Process using Multi-agency Review Meetings - Update CAF where necessary - If any changes update the Information Sharing Project Team
