This project aims to improve heart failure management in skilled nursing facilities by implementing educational programs and tools for staff, patients, and families. Results show the need for tailored disease management programs, dietary flexibility, and medication protocols. Collaborative efforts focus on reducing readmission rates and enhancing symptom recognition. Supported by healthcare professionals, the initiative strives for better patient outcomes and quality care.
