Preop Patient Check off • No matter what the situation when collecting a patient for surgery take the time to… FOAM IN! Validate with preop/ER/ICU nurse and the patient if they are able.. • the patients full name and DOB and ensure they match the name on the blue card and arm band • The medical record number matches blue card and armband
Preop Patient Check off • Verify the allergies bracelet! • Verify with the patient that the procedure matches OR schedule! • Verify the consent for the procedure & the site initialled by proceduralist if a lateralized procedure! • Verify the site, size & patency of the IV and the antibiotic order matches the antibiotic hanging!
