    4. Annual Site Visit Update All site visits are completed! Reports in process. Make sure your program: Replaced all forms – SR forms are all being used Grievance policy on the Family Rights & Confidentiality Documenting supervision of supervisor/manager All training is updated in training tracker Home Visitor Goal Plans are in place Document on the Home Visit Record if a Parent Child Activity is not included on a home visit Has sent revised PPPM to OCCF – was due in Oct. Program Goal Plans to OCCF within 45 days of receiving Site Visit Report

    5. Family Manager Release 2 Goals of FMR2: To get you the data you need for local planning and action! To get local and state data we all need for HFA Re-Accreditation To manage performance indicators and FSUs from both the state and local level Timelines: Home Visit Completion from July 2010 will be entered by each program once FMR2 is released (~January 1, 2010) ~January 1, 2011 – Programs begin entering Exit/Re-Entry data (no longer sending to NPC Research) July 1, 2011 – Programs begin entering NBQ and KEMPE data (no longer sending to NPC Research)
