  2. There are 6 parts to the PAD • Move to electronic portfolio through Faculty 180 in progress but not here yet. The Personnel Action Dossier (PAD) is a collection of documents and lists of accomplishments in summary form that provides a record of the accomplishments of a faculty member seeking reappointment, promotion, or tenure. A PAD is compiled in a manner described in The Faculty Manual and is submitted each time a personnel action for reappointment, promotion, or tenure takes place. Each PAD becomes part of the faculty member’s permanent personnel file and is not returned to the faculty member. There are 6 parts to the PAD • maximum of 4” binder for contents • make a copy before you submit • attention is paid both to productivity since the date of hire, tenure or last promotion (whichever is more recent) and accomplishments over one’s entire career. ECU is in the process of evaluating systems for the eventual use of electronic PADs. In the meantime faculty are encouraged to store electronically materials needed for the PAD and must submit a paper PAD using a binder no larger than 4 inches. In Spring of 2017, faculty were urged to take documents from Sedona, the previous software, to store elsewhere during the transition.

  5. Parts of the PAD – Tab 1 • Tab 1(A) - The Cumulative Report. (Some units also require CV) Properly executed ECU Cumulative Report for Reappointment, Promotion and Tenure (see form) is required for these personnel actions. It is the responsibility of the faculty member to have prima facie evidence of all activity listed in this report available for inspection, if requested, by reviewers at any level of the personnel action process. Actions involving tenure and promotion also require a properly executed ECU Personnel Action Summary Form. [B5 of the Cumulative Report says: “Summary of teaching evaluations (Student Perception of Teaching Survey or Student Opinion of Instruction Survey Results, peer reviews, and any additional supporting information.)”; summary data should be here, without condensing or interpretation. The Chair of the Faculty and Provost memorandum of January 12, 2017 directs the following pertaining to the SSOI reporting: “At present, raw SSOI response data will be included in the PAD in the student opinion of instruction survey section. (i.e. section B.5 of the cumulative report, Faculty Manual Part X). Data should not be condensed or summarized, but included as it appears in the original survey report. The student comments are not included.” The full memo is available on the OFE website under Evaluation.] * If it becomes difficult to include full survey reports within the Cumulative Report, it is acceptable to move these to an Appendix within Tab 1(A) or to Tab 4C - Supporting Materials. Just ensure that all student and peer evaluation materials are available within the PAD.

  6. Parts of the PAD – Tabs 2, 3 • Tab 2 (B) - The Recommendations (Note reappointment, tenure and promotion information) . The documents are added by the appropriate official as the process progresses. • Tab 3 (C ) – Record of Evaluations • Criteria for tenure and/or promotion set forth by the code unit • Copies of all written communications with the candidate on progress toward tenure, including annual evaluations over time period (Progress toward tenure letters and other personnel committee communications, etc.). May be copies of emails. • Records of annual evaluation meetings with unit administrator • Records of assigned teaching duties and responsibilities, including reassigned time • Records of annual discussion on criteria for evaluation of faculty performance • For final tenure/promotion decision, copies of communication with external reviewers and copies of external reviews, listing of documents reviewed.

Parts of the PAD – Tab 4Section on Teaching • Tab 4 (D) – Supporting Materials (suggestions) • Table of Contents - for whole section or one for each of three or more parts: teaching, research, service, patient care • Section on Teaching – • Philosophy of Teaching (one or two pages; maximum of three) • List of courses taught, perhaps with a brief description of challenges/ rewards & terms taught • Objectives of teaching • Example(s) of course syllabus/syllabi • SOIS/SPOTS/Blue (SSOI) or other student input reports – Explain any aberrations and steps you have taken to address identified areas for improvement; note difficulty level and response rate if appropriate • Example(s) of special assignments, projects, and/or other activities • Samples of student work evaluated; self-made student evaluations used • Example(s) of unsolicited notes or other feedback from students • Faculty development activity and reflection on growth (course redesign, problem-based, team-based, DE course, service learning, assessment efforts)

  8. Use the philosophy of teaching statement to (a) demonstrate that you have been reflective and purposeful about your teaching (b) communicate your goals as an instructor and your corresponding actions in the classroom, and (3) provide an opportunity to point to and tie together the other sections of your portfolio (ftad.osu). Discuss the breath and/or depth of your work? What have you done/plan to do?(philosophy of research and creative activity) • Selected example(s) of research and creative activity – report(s), article(s), photographs of work, link to materials • Audience and impact – journal selection and quality, citations, invitations, other examples of impact • Overview of grant work and impact – letters of awards, letters where no funding but positive content • Scholarship of Discovery • Scholarship of Teaching and Learning (SoTL) • Scholarship of Engagement

  10. Parts of the PAD – Tab 4 (D)Section on Service Section on Service Philosophy of Service and how you have pursued it; consider connections among teaching, scholarship, and service Examples of service accomplishments – professional (national, university, department), community, etc. (Servire Society, leadership, recognition) Letters, materials, student comments, etc.

  11. Parts of the PAD – Tabs 5 (E), 6 (F) Tab 5 (E) – Other Material - information not related to this specific position that you believe is important to your achievement. This section may have nothing in it. Tab 6 (F) – Disagreements. This section may have nothing in it.
