  2. Patient History 56 year old white female Smokes cigarettes (pack/day) Is not on any medications Has no known health complications Seeking regular dental care

  3. Lesion Description Red single localized macula approximately 2 cm in an anterior/posterior direction and approx 1 cm laterally. The lesion is located in the posterior midline on the dorsum of the tongue anterior to the circumvallate papillae. The surface is devoid of filiform papilla but does have a nodular appearance. The borders are well-defined and the lesion is epithelium involved.

  4. Patient Dialogue Have you ever noticed this before? Have you ever been told about this lesion? Do you suck on cough drops, aspirin, etc? Do you recall any traumatic occurrence that may have caused this lesion?

  5. Differential Diagnosis Median rhomboid glossitis Geographic tongue Chemical burn Traumatic ulcer gumma of tertiary syphilis

  6. Provisional Diagnosis Treatment and Prognosis No treatment is recommended for median rhomboid glossitis. Although sometimes the nodular form, as opposed to the smooth flat type, is biopsied to confirm that the lesion is benign. If a biopsy is to be done the pt should first be put on an antifungal agent to reduce inflammation and erythema for a more accurate pathology assessment. Antifungal therapy is also sometimes utilized to treat any candida infection that may be present. Some common antifungals are nystatin, clotrimazole, fluconazole and ketoconazole.
