  2. Strategy of the Health Reform, is to modernize and improve PHC servicesthrough the implementation of a community-oriented Family Health Team (FHT) approach. The FHT approach represents a system of delivering primary health care through a multidisciplinary teamof health professionals that works together to serve the comprehensive health needsof a definedpopulationacross the life cycleand in a community setting that is closetothe client The phrase “My doctor / my patient” captures the essence of the FHT approach. FHT will improve workload by managing demand through appointment and triage systems and by making the best use of staff skills and time through task shifting. Reform in Jordan – Achievements • Patient-related health information system established and in operation at 6 health centers (e-health). • Pharmacy Module produced and piloted and will be replicated at 18 health centers that are not part of the e-health. • Hospital Referral prototype piloted at four Area Offices and two health centers. • Health center appointment system improved and implemented with the aid of Q-System. • UNRWA’s active participation in health policy forum institutionalized.

Next Steps • Appointment System implemented at station level in 6 health centers. • Team work strengthened through training and task management. • Hospital Referral System implemented at 18 remaining Health Centers. • Clear ToRs and Service Level Agreements for e-health between stakeholders. • Work on extracting all needed reports from the e-health system. • Pharmacy module to be installed in remaining 18 health centers. • Integrated Q-system with the e-health to be piloted and tested at Nuzha Health Centre then implemented in the remaining five health centers. • Health Information System functioning in the four Areas. • Building on task analysis and rebalancing the roles and responsibilities of all staff to make efficient use of human resources.
