GP → Out of Hours Palliative Care Handover Form • South doc after hours family doctor service are piloting this GP → OOH handover form. • The design is based on the HSE GP Referral form, and the content is informed by the findings of GP research in Cork (Kiely 2010), and the existing form used in Scotland • The pre-pilot phase commenced in Oct 2012, involving GPs who are registered in Cork and Kerry (approx 600) and use South Doc services. • Copies of the form and information material available on the IHF website – Primary Palliative Care page. • Dissemination and Synergies • The work of this programme was presented at the International Primary Palliative Care Research Group in Montreal, along side speakers from the UK, Canada, Australia. • Posters on the findings from the report ensuing work have been presented at ICGP, HSE and palliative care conferences. • Articles on this work have been published in the ICGP Forum Magazine, The Practice and World Irish Nursing • 2. Identification of Patients with Palliative Care needs • The identification of those people with palliative care needs in primary care settings is the first step in their appropriate assessment and treatment. • Such identification will assist in service planning and communication with both patients, their families and staff. • The Supportive and Palliative Care Indicators Tool (SPICT) was determined by the committee as the most suitable tool to use for this purpose • A project proposal is being prepared to pilot this tool in Primary Care settings in Ireland, based on evaluation of use of this tool in UK settings.
