California LifeLine Customer Interactive Website Demonstration • Interactive Website Demonstration • New refreshed look and feel of entire website • Enhancements keep the existing paper process, adding the on-line enrollment option • The new site is designed as similar to the paper program as possible • Website designed for Section 508 Compliance and text to voice screen reading software • Certification demonstration • Verification demonstration • New ‘Remove Me’ feature for online applications

Example of PIN as found on the front of the application • PIN will be auto-generated for each customer • PIN will be placed on the front of each application • PIN will be retrievable via the Call Center in the event a customer looses the application • Beginning on or about May 5th 2008, all English language Certification forms will be sent with a PIN to new customers • On June 1st 2008 the PIN will be printed on all language Certification and Verification forms
