  2. Purpose • To inform and explain to the TSM the importance and time frames for the Separation History and Physical Examination (SHPE) and to include the health and mental health care options available to themselves and their families. 1

  3. Separation History and Physical Examination (SHPE) • NOW a requirement for ALL separating AD service members. • Comprehensive medical evaluation to include dental. • Scheduled 180 days (6 mos.) out prior to your separation/retirement date. • Ensure any medical condition requiring ongoing care are • identified and documented in the military medical record. • Reserve and National Guard who have served a minimum • of 180 days on AD or 30 days in a contingency must • complete the SHPE prior to release from AD. 2

Separation History and Physical Examination (SHPE) cont’d • Will help establish service connection for any medical • conditions that have developed if planning to apply for VA benefits, whether now or in the future. • If applying for VA benefits through the Benefits Delivery at • Discharge (BDD) program, the exam received from the VA will meet the SHPE requirement. • - If not applying for VA benefits or not completing BDD prior to separation, member will receive the SHPE through the Military Treatment Facility (MTF). 3

  5. SHPE Items • Lab review and verification (HIV draw within 1 yr.) • Exams required: Dental, Eye and Audio/Hearing. • Immunizations review and verification. • DD-form 2807-1 / Report of Medical History. • DD-form 2808 / Report of Medical Examination (completed/signed by MO). 4

  6. SHPE Additional Items • DD-form 2697 / Report of Medical Assessment. • SF-600 PQ/NPQ (signed by MO). • NAVPERS 1070/613(Page 13) / Medically Cleared • NPPSC Sep Questionnaire/Checklist 1900/1 • 1900/2 4

  7. Mental Health Services • DOD Mental Health Self-Assessment Program • Mental health and alcohol screening and referral program provided for service members and families affected by deployment and mobilization. • It is a voluntary and anonymous program. • AD service members can self-refer for mental health • if seen at a MTF. 5

Mental Health Services cont’d • - Active duty family members seeking mental health appointments, the first 8 visits do not require preauthorization if seeking care within TRICARE. • DOD InTransition Program • Confidential coaching program that answers • questions about treatment options, provides community resources and assist with appointing a • behavioral health provider at your new location. 6

Mental Health Services cont’d • Veteran’s Affairs (VA) • Specializes in treatment of mental health issues, such as PTSD and substance use disorders. • Pharmacotherapy, psychotherapy, group and for opiate use disorder, medication assisted treatment. • - VA provides a toll-free, confidential Veterans Crisis Line for veterans and their families and friends. 7
