  2. Report HSB emergencies to 911 • Report accidents and incidents to your supervisor and to the Dept. Administrator: Dima Long (616-4218; dimalong@uw.edu ).

  3. In case of alarm, follow these evacuation procedures: • Quickly exit the building through the evacuation routes; • Assemble with the rest of the employees at our designated meeting location (primary and secondary locations): • Primary: across from the G-Loading dock; • Secondary: at the parking lot across from the South Campus bldg entrance with the ATM machine. • If you have pulled the alarm, meet with the Dept. Safety warden and report the problem. • Wait for a signal from the safety warden to re-enter the building.

  4. Fire alarm pull stations locations: see next slide. • If you discover a fire: • Pull the nearest fire alarm • Exit the alarmed area • Proceed to evacuation meeting area. • When you hear the alarm: • Leave the building • Close your door behind you.

  5. There are 3 portable fire extinguishers in the H-Wing, 2nd floor: see slide. • You should not utilize the fire extinguisher unless: • You have been trained how to use it • The fire alarm has been sounded first • The fire is small (waste basket size), and • You have a clear evacuation route.

  6. The First Aid kits for the H-Wing, 2nd floor are located at: • H-268 (Unadkat lab) • H-265 (Wang lab) • H-253 (Isoherranen lab) • H-159 (Kelly lab) • H-259 (Mao lab) • Pharmaceutics Front Desk area • Closest safety shower locations: see next slides (Tauri) • Closest eye wash stations locations: see next slides (Tauri)

  7. Once you report on-the-job accidents to your supervisor and the department administrator, you are required to fill out a University accident incident report form: http://www.ehs.washington.edu/ohsoars/index.shtm

  8. All automobile accidents in University-owned vehicles must also be reported to the University Police Department (911) immediately, whether or not there appears to be personal injury or property damage.

  9. In addition to reporting unsafe conditions and acts to your supervisor and/or the department administrator, you may choose to report safety problems to the Environmental Health & Safety Office.

  10. Work-related injuries or illnesses resulting in medical expenses or time loss are covered by the Washington State’s Workers’ Compensation. • To establish a Workers’ Compensation claim, employees must fill out a State Department of Labor and Industries (L&I) Report of Industrial Injury or Occupational Disease at their medical provider’s office when they receive medical care for a work-related injury or illness. • Prompt reporting of accidents to your supervisor, will make the claims process easier and may allow him/her to find you modified work during your recovery.

  11. Hazardous materials are used in all Pharmaceutics departmental laboratories. • All chemicals must be labeled with the chemical name and amount or concentration of chemical in the container. • 1˚ container (Original Container) • Do not remove or deface original label • If original chemical is altered, place a label or piece of tape • Replace original label if illegible • 2˚ container (If original sample is transferred or diluted) • Label container with chemical name, concentration, name of person who transferred sample and date of transfer

  12. Material Safety Data Sheets (MSDS) are available online through the MyChem database. Gotohttp://www.ehs.washington.edu/ • Click on “MyChem” link • Click on Access the MSDS collection (if you have UWNetID) • Complete access form • MSDS tells you flammability, reactivity, toxicity, etc. which determines handling, storage, and disposal techniques.

  13. Report small hazardous materials emergencies to your supervisor immediately. • Large spills or releases call 911. • Accidental needle sticks or exposure, call UW Employee Health Center 206-685-1026 or 206-744-3081 (if at Harborview) Use your judgment, but error on side of safety

  14. Hazardous material must be stored in the appropriate places, see MSDS or supervisor • Organic solvents - yellow flammable cabinets under fume hoods or biological safety cabinet • Strong acids and bases must be stored separately in the acid and base cabinets, respectively

  15. Hazardous waste must be saved in a container labeled with the name and amount of each component. • When “Hazardous waste” container is full, a routine chemical collection request can be made at the following link: • http://www.ehs.washington.edu/forms/epo/routinepickup.php • Or a collection request form can be printed and mailed: • http://www.ehs.washington.edu/forms/epo/1957.pdf

  16. General safety orientation is provided to all new employees by the Pharmaceutics department. • You must be aware of all rules in the lab which are documented in the laboratory safety manual. • Your lab manager should also advise you of training, apparel, procedure, etc. requirements.

  17. New employees should be registered for applicable EH&S training courses as close as possible to start date. • You must receive training appropriate to the procedures you perform: • Example – If you work with human blood and/or tissue, you must take the “bloodborne pathogens …” training • To determine required training, goto: • http://www.washington.edu/research/compliance/required-training/ or • http://www.ehs.washington.edu/ • click on “Training Information”

  18. These are discussed by supervisors and lab managers for each individual lab at your lab safety orientation and safety lab meetings.

  19. Check with your supervisor to find out exactly what type of protective equipment you need for your job. Examples of PPE are below. Your supervisor should explain to you the precise use, care, cleaning, and storage of any personal protective equipment that you will be required to use on the job. The University mandates strict adherence to all department and lab policies on the use of PPE.

  20. The University is divided into eleven organizational groupings, each one represented by an OrganizationalHealth and Safety Committee. This committee deals with issues the members may have in common but can handle more effectively together. Each elected member represents all units of that organizational group, including his/her own. • Our department is currently represented on the Group # 4 Organizational Health & Safety Committee by: Diana Shuster (elected) and Eric Kantor (appointed). • The (Group #4) Committee reports to the following executive – Bob Ennes.

  21. Our safety bulletin boards at HSB are located in the kitchen area and in the front office area in H-272. Roosevelt and Western Ave. facilities also have safety bulletin boards located at their common areas.

  22. Individual attacks on faculty, staff and students can and have occurred due to domestic violence or workplace violence. As part of maintaining a healthy, safe working environment, the University has developed and administers one UW Violence in the Workplace Policy and Procedure through the Human Resource's Violence Prevention and Response Program: http://www.washington.edu/admin/hr/polproc/work-violence/index.html . • University services include nighttime safety escort services, counseling sessions, a dedicated assessment team, and informational materials and training, but services are not limited to these items.   • All managers, supervisors, and employees must be aware of the appropriate processes to follow regarding workplace and domestic violence prevention. They can receive assistance in answering any employee questions from the HR Violence Prevention and Response Program Manager. We expect our entire faculty and staff to take Workplace Violence training at least once every biennium, as well as receive information during new employee orientation. •  For more comprehensive information, access the SafeCampus website at http://www.washington.edu/safecampus .   • If any staff has concerns regarding a threat of violence, call:   .   23. No smoking inside the building at any time (see additional info at http://www.ehs.washington.edu/psosmoking/index.shtm ). • Alert your supervisor if you are planning on working alone after hours. Make sure someone at home or at the lab knows about your late activities. Keep all lab and office doors closed and locked at all times. • Use caution when walking around campus during dark hours.
