  2. ETT/TransNetsmUpdate • History • ETT – TransNetsm naming the service • TransNetsm , version 5.0 highlights • Train-the-Trainer sessions • Beta test • Next steps

  3. HHS Entrepreneur Program - Project Goals

  4. A Collaborative Effort • HHS – Entrepreneur Program • HRSA – David Cartier, Chris McLaughlin, Funding • UNOS– John Rosendale, Amy Putman, IT, Instructional Innovations, Communications • OPTN Ad Hoc Organ Tracking Committee • OPTN Operations and Safety Committee • OPO and Transplant Center Staff • External Barcoding Expert • Patient Safety Program and Simulation Center– Northwestern University • Virginia Commonwealth University Simulation Center • Apparatus Foundry

  5. Discovery - Development • OPO Deep Dive • Transplant Centers • FMEA • Rapid Cycle • Develop • Simmulation • Develop • Simmulation • Devlelop • Simmulation

  6. Organ Validation, Labeling and Tracking System

  7. Field Testing • Included training, proficiency testing, testing on actual donor cases, and 24/7 support from ETT team

  8. Survey Results • A survey was submitted after each donor case by anyone using the system in either the ICU (donor management phase) or the OR (surgical recovery phase). • Surveys received as of April 6, 2014

  9. The Field Test donor management (ICU) process with the tablet application is SAFER/MORE EFFICIENT than the current process. Mean = 4.4 Mean = 4.5

  10. The Field Test OR process with the tablet application is SAFER/MORE EFFICIENT than the current process. Mean = 3.8 Mean = 4.1

  11. ETT - TransNetsm - Naming the Service • Many requests from field test participants to change the name. ETT can cause confusion because in the hospital setting ETT is an abbreviation for endotracheal tube • “TransNetsm – a Service of the OPTN” was chosen for several reasons • Net - Although TransNetsm is a separate service with a separate login, it interacts with DonorNet throughout the process • Trans • Gives a feeling of movement/action. This is a service used in a very active process. • Implies, without naming, Transplant, Transport, Tracking, etc.

  12. TransNetsm - Highlights • Many changes made to the service based on feedback from the field test participants • The following slides highlight a few of these enhancements/changes/additions

  13. TransNetsm - Highlights • Authentication • Username and password that associate user with their DonorNet permissions • Allows communication with DonorNet • Requires anyone using the application to be certified by their OPO • Associates all actions with a user (Audit logs)

  14. TransNetsm - Highlights • Creating a case • When on WiFi, communicates with DonorNet to pull in donor information and serology results • When no WiFi available, case can be created manually

  15. TransNetsm - Highlights • Validating data • Coordinator has the option to validate the entered data either locally or remotely • Individual field validation associated with validator

  16. TransNetsm - Highlights • Printing blood labels • Allows coordinator to print blood labels with or without ABO • Allows for multiple draws

  17. TransNetsm - Highlights • Validating infectious disease results (for the vessels label) • Can be done locally or remotely • Individual field validation associated with validator

  18. TransNetsm - Highlights • Procurement Plan • Allows coordinator to choose organs that are planned to be recovered • Print documentation labels for the organs chosen • Organs chosen follows through to the Procurement page

  19. TransNetsm - Highlights • Specimen Labels • One page to print additional items that accompany organs • Nodes • Spleen • Nodes & Spleen • Vessels • Biopsies • Other

  20. TransNetsm - Highlights • Procurement Screen • Only includes organs that were chosen on the Procurement Plan Screen • Press one button to print all of the Organ labels • Press one button to print all of the Shipping Label 1 labels

  21. TransNetsm - Highlights • Procurement Organ Detail Screen • Clearly identifies the organ being packaged • One place to: • Scan contents into the box • Enter ice time • Print Shipping Labels 2-3-4 with one button • Final scan of Shipping Label 4 in application

  22. TransNetsm - Highlights • Scanning into the box • Continuous scanning • Running log • Quicker scanning

  23. TransNetsm – Highlights – Additional Changes • Repackaging functionality • Transfer of case via WiFi • Transfer of case via Bluetooth • Web application for reports, audit logs, managing users • Many other smaller enhancements

  24. TransNetsm – Train-the-Trainer Sessions • UNOS Headquarters on September 10-11, 2014 • Two representatives from each of the beta test sites • Learned the new system • Passed competency certification test • Received training on being a trainer • Access to SharePoint site with tools for training • Return to their OPOs to train and certify their staff

  25. TransNetsm – Beta Test - OPOs • Eight OPOs – September 18, 2014 – January 15, 2015

  26. TransNetsm – Beta Test - OPOs • Once a coordinator has been trained and certified, the TransNetsm service is used in actual donor cases (ICU and OR) • Short follow-up survey following each case • Support provided by the TransNetsmteam, Help Desk, and Organ Center • Sites prepared to revert to manual process if needed • Feedback used to improve the service

  27. TransNetsm – Beta Test – Transplant Centers • Transplant hospital responsibilities • Print recipient ID band from DonorNet • Scan receipt of the organ package when it arrives at the transplant hospital • Scan to match the organ to the recipient • October 2014 – January 2015 • Proof of concept test • Monthly feedback to the TransNetsm team • Center must be prepared for both TransNetsm packaged organs as well as organ packaged and labeled using the current system • Feedback and discovery to inform and shape the future

  28. Print TransNetsm Recipient ID Band • ID Band printed from offer screen in DonorNet • Contains human readable information and barcode • Human readable • Recipient name, DOB, ABO, Donor ID, organ(s) anticipated • Barcode • Donor ID, Donor ABO, Organ

  29. TransNetsm Website

  30. TransNetsm Website – Organ Receipt • Upon receipt, the barcode on the outside of the organ container is scanned • Information recorded • Donor Information • Receiving Transplant Center • Location • Received by • Any comments

  31. TransNetsm Website – Organ Match • In the transplant OR • Organ is scanned • Recipient ID Band is scanned • System respond with the result of the scan

  32. First Transplant Center Use of TransNetsm • Piedmont Hospital in Atlanta, GA (Beta test participant) • On October 9, 2014, became the first transplant center in the country to use TransNetsm system in an actual case. • 19:30 – Printed and attached the Recipient ID band • 19:54 – Scanned receipt of the liver organ container • 21:59 – Successfully scanned and matched the liver and the recipient

  33. TransNetsm – Next Steps • Enhancements to Android application • Based on feedback from beta test • UNOS IT/Vendor • Voluntary Nationwide deployment • Train the Trainer sessions • Multiplatform application • Refine the transplant hospital functionality • Based on feedback from beta test • Discovery project • Hope Act
