  2. Community Health Needs Assessment WHAT IS IT? Purpose: Identify and describe factors that affect the health of a community, and identify available resources to address those factors. Process: Form an advisory committee Collect and review data Select 3 priority health issues Identify future partners/resources Develop a written implementation strategy Timeline: Began September 2018 Ending May 2019 Top 10 Priorities identified: 1. Mental Health 2. Stress 3. Depression 4. Chronic Disease 5. Suicide 6. Obesity 7. Transportation 8. Substance use 9. Access to healthy and affordable food 10. Access and awareness to services and resources & dental care (tied)

  15. Community Health Improvement Plan WHAT IS IT? Purpose: A vision for the community’s health that addresses long-term, systematic efforts to address public health problems within the community. This is the community’s plan not the community health boards plan for the community. Process: Prioritized issues with advisory committee Narrowed down to top 3 priority areas with advisory committee Conducted a root cause analysis of top 3 areas Currently developing a structure where the root causes are the final priorities. Next Steps: Finalize structure, formulate goals, strategies, and define roles with partners Implement, monitor, and revise with partners Timeline: Began February 2019 End date March 2020
