  3. Any Accidents or Injuries…? PLEASE REMEMBER TO REPORT IMMEDIATELY !! Reporting injuries immediately is the optimal way to save time, money, and your associates!  If you have questions please contact Risk Management at 757-989-2724. Workers' Compensation claims must be reported online,  via HR Manager Self Service. Send supporting documentation for claims to 757-989-2501 OR accident.claims@ferguson.com
  4. Reporting Accidents.. All vehicle accidents should be reported to the Risk Management Group immediately utilizing the on-line reporting tool on Pipeline, or by calling (757) 989-2724. If there are injuries to our associates, also notify Liberty Mutual 877-308-8141 within 24 hours. FYI (California locations) - Please note additional forms for submittal of a workers’ compensation claim found on pipeline. Does your truck have an Accident Kit?

  7. FY-2013-14 SAFETY INCENTIVES As leaders we are entrusted with providing a safe work environment for our associates. We have a united philosophy that the majority of accidents are preventable. To this end, Risk Management announces the following program regarding Safety Incentives for fiscal year 2013/2014:

Branch Operational Audits If your site is selected for the Branch Operational Audit, and you receive a passing score (green) for both the Health and Safety and Transportation sections, your location will automatically receive 100%of the incentive. FYI – ONLY 7 out of 27 locations recently audited PASSED this section…
  9. Incentive Criteria (1): Behavior Based Safety Activity = 25% of available pool Implement and document the Behavior Based Safety activity. The goal of BBS is to create a culture of safety through awareness. We know that trucks and equipment do not cause accidents; people do by engaging in unsafe behaviors.
  10. Incentive Criteria (2): Safety & DOT Combined = 25% of available pool Consistent with last year, the Safety and DOT assessments are combined into one document. Access via the following path: Pipeline > Risk Management > Safety > Incentives Locations are required to complete formal evaluations every six months (2 per fiscal year). Location management is expected to perform informal evaluations monthly. Documentation must be kept on file at each location for review by management and/or regulatory agencies. For purposes of the incentive, one assessment will be submitted via on-line survey each fiscal year.
  11. Incentive Criteria (3): Timely accident reporting = 25% of available pool A direct relationship exists between the length of time to report a claim and the escalation of cost. All workers’ compensation claims should be reported on-line through Peoplesoft within 24 hours. All vehicle accidents must be reported on-line via the following path within 24 hours – Pipeline > Risk Management > Claims > Vehicle Accidents > Report an Accident
  12. Incentive Criteria: (4) DOT Citations = 25% of available pool Locations receiving less than 2 DOT Citations during the fiscal year will qualify for this portion of the incentive. All defects noted during a roadside inspection are considered citations, regardless of whether a monetary penalty is assessed. All citations affect the company as a whole, regardless of where they originate.

Hours of Service Regulation Change Effective August 2nd, the FMCSA has vacated* the 30 minute rest break requirement for all short haul operations.  This means that if your drivers do not exceed a 100 air mile radius (from the location), they are no longer required to take the 30 minute rest break.
  15. Hours of Service Regulation Change Action: ONLY drivers that exceeds a  100 air mile radius (from their location), must follow the 30 minute rest break.  The rest break must be one break with a duration of at least 30 minutes and be indicated on the log graph as off duty time.  ‘In attendance’ time applies to drivers with trucks carrying Hazardous Materials, where the driver is required to ensure the truck is not left unattended.

Hours of Service Regulation Change The vacated* regulation will not officially go into effect for about two months, but the FMCSA has requested that state agencies cease enforcement of the regulation.  As a result, states should not be citing drivers for the rest break, as long as they do not exceed 100 air miles.  If your driver does receive a citation and is within a 100 air mile radius (from the location), please contact Amanda Rose or Risk Management Dept immediately so we can challenge the citation and have it reversed and removed from our record.

Fall Protection – Order Pickers Any associate certified to operate an order picker must wear a full body harness at all times and be attached to a personal fall arrest system BEFORE operating the equipment regardless. i.e.… As soon as they step on the equipment the first thing they must do is attach the fall arrest system to their full body harness. * Belts and lanyards are not permitted in Ferguson Locations.

Baltimore, MA RECEIVED PASSING GRADE FYI - All results are considered privileged and we do not want to share those results outside of FFF.  The audit group is very protective of branch results and while Baltimore passed, it is very possible that we could have branches receiving a failing grade.

Sections failed.. 1.2 - Are copies of deposit slips and checks attached to Cash Reconciliation Report? 1.10 - Is the deposit slip approved by a different associate than the preparer? 1.14 - Is the Cash Reconciliation Report initialed by an associate other than the preparer? 2.3 - Are Tax Exempt certificates on file for customers whose sales were tax exempt? 6.3 - Are all entry points to the branch yard secured?

Sections failed.. 6.9 - Are vehicle fuel cards properly assigned and secured? 8.1 - Is sensitive Information (e.g. SSNs, CC#s Health Info) properly safeguarded? 10.9 - Are areas that could present a fall hazard adequately secured (e.g. stairs, mezzanines)? 10.12 - Does evidence of new hire safety training exist? 10.17 - Does evidence of an active Safety Committee exist and are minutes posted?
