Update • Variation reduction • 51% reduction in variation • 4% reduction in admissions • 14% reduction in referrals • RECOGNISED: across Derbyshire and SHA for unwarranted clinical variation reduction • CONFIDENCE & EXTRA MONEY: getting extra money to help us continue work & new support pathways BETTER CARE . BETTER HEALTH. BETTER VALUE .

What Happening? • New pathways & services planned: • Now: Matrons & nurses changing work patterns e.g. rightcare plans • Tomorrow: Additional Pharmacy support – Interviewing • June 2012: AF Screening & monitoring equipment • June 2012: IT Links for information from NUH • July 2012: Ophthalmology service – expected July 2012 • Start September: Integrated care – health and social care colleagues working with GP Practices • October 2012: Single point of access for urgent care being created • October 2012: LUTs pathway • All year: Urgent care information validation (local CQUIN), pathway changes, variation reduction • Other areas being considered: Rheumatology

CCG Application • What’s happening when? • Application on 1st October 2012 (Wave 3) • Collation of all information to meet the application being managed by Management Team of Officers and Lead GPs. • Information includes: • CCG Constitution (will be discussed in next QUEST) • Organisational development plan • Engagement and Communication plan • Commissioning & Business Plans • Governance documentation • Financial Planning documentation • Authorisation process: • 360 degree review by our stakeholders • Desktop review by DH of our application and information • DH will interview the CCG team of Lead GPs and Officers
