Rollover of Truck-Tractor Semitrailer and Collision with Motorcoach on Interstate 94, Wisconsin

On October 16, 2005, a serious accident occurred on Interstate Highway 94 near Osseo, Wisconsin involving a rollover of a truck-tractor semitrailer and a collision with a motorcoach. The investigation, led by David S. Rayburn and a team of experts, examined factors including highway conditions, vehicle performance, operator fatigue, and survival factors. The truck driver, exhibiting signs of severe fatigue and having slept less than 5 hours prior to the incident, caused the vehicle to drift from the right-hand lane at approximately 1:58 a.m.

  19. Truck Driver Fatigue • At most, the truck driver obtained 12.5 hours of sleep in the 2 days before the accident • Less than 5 hours of sleep in last sleep period • Was awake for at least 15 hours before the accident • Sleep was disrupted by alcohol use • Accident occurred at 1:58 a.m.
