  4. Doctors’ offices are Changing YOU MAY FIND: • “AFTER HOUR” APPOINTMENTS • CARE COORDINATORS • INFORMATION ABOUT COMMUNITY RESOURCES • INFORMATION IN YOUR OWN LANGUAGE

Medical home care coordination Practice-Based Care Coordinators Not always done by the physician • Makes sure specialists and primary care doctors are communicating • Coordinates appointments • Is aware of, and shares with you community resources • Communicates with schools, mental health, dental • Refers you to other families for peer to peer support

Doctor’s offices are changing You may experience: • More questions about your child’s behavior • More questions about your social living situations • More questions about mental health • More “screening” questionnaires about your child’s development • More questions about your child’s school experience

  7. Your level of participation Family Centered Care requires family input! Moving from “They told me…” to “WE decided…” • Expect to share with your doctor what your child does well (his strengths) • Expect to share your concerns • Expect to participate in decision making and planning care and treatment • Be prepared before attending a well child visit!

  8. GOAL: PARENTS SHOULD FEEL MORE CONFIDENT CARING FOR THEIR CHILDREN WITH SPECIAL NEEDS AFTER AN APPOINTMENT, NOT LESS CONFIDENT!
