Duties of Individual Being Graded • Provide brag sheet • Complete brag sheet to provide information to your grader • Provide examples and be quantitative if possible • Provide Dream Sheet • Be familiar with the FITREP form IOT provide the appropriate information • Include if you are not in standards • Provide swim qual/sail qual information

Duties of Primary Grader • Ensure you receive brag sheets • Ensure you receive dream sheet and discuss with the individual being evaluated • Provide your people with a timeline • Complete FITREP correctly and completely • Your chain of command should not have to go behind you and correct any mistakes.

  6. Brag Sheet Instruction Name: LAST, FIRST, MI, SUFFIX Term: Fall 2011

Brag Sheet Instruction Academics • Major: list major or write undeclared. • Minor/Certificate: include minor, or list N/A • Navy options should include Naval Science • Achievements: • DO NOT: • list grades from last semester, guess grades for current semester or write none • DO: • provide information on school work • goals discussed with advisor, • include GPA if you achieved a specific status • 3.7 GPA semester Dean’s list • list academic awards from last awards day

Brag Sheet Instruction Battalion • Battalion Duties/Responsibilities • DO NOT: leave blank • DO: write MIR or specify official duties • squad leader, guide, etc • include billets that are not company related • academics chief, recruiting officer, etc • Awards: • DO NOT: leave blank • DO: write N/A if you have none or list any ribbons, medals or commendations you received • MIDN of month, leadership award • Other Achievements: • DO NOT: leave blank • DO: write N/A or include any battalion related activities • drill team, intramurals

Brag Sheet Instruction Physical Fitness • Achievements: • DO NOT: leave blank • DO: include improvements in PRT scores • quantitative information is always best: • improved by 6 pushups, dropped 23 seconds on run time, etc • include PT awards, provide information on what you do to stay fit • explain your workout routine

  10. Brag Sheet Instruction Extra Curricular • Achievements: • DO NOT: leave blank • DO: provide information to your grader on what you do outside of school and NROTC. • This could be club sports, an academic club or society, volunteer or church involvement, a job, etc

FITREP Instruction Block 1: all capital letters • LAST, FIRST, MI, SUFFIX • Ex: SMITH, JOHN, D III • If someone does not have a middle name, NMN should appear in the MI position • Ex: DOE, JANE, NMN Block 2: 201X Block 3: all capital letters in company format • This block will be completed as one of the following: ALPHA, BRAVO or BN STAFF

  13. FITREP Instruction Block 4: Spring Block 5: completed with appropriate date. (DDMMMYYY ie 10NOV1775 ) • This block will be completed as one of the following: Block 6: regular will be checked Block 7: date will be in same format at block 5 • Marines will have number format (###) • Navy will have one of the following: SM SH GL GM GH EL EM EH OL OM OH • Or F for failure, P for probationary, M for medical waiver • Individual being graded will provide graders information if not within standards Block 8 and 9: information to be provided by individual being graded (MON YEAR)

  14. FITREP Instruction Block 10: • a: NROTC billets or MIR • b: major (NOT in all capitals) or undeclared Block 11: NROTC awards listed first, then university honors. If none, then write N/A Block 13: enter name of active duty advisor in all capitals in RANK LASTNAME format • Ex: MAJ LANCASTER , LT BURICH, etc

  15. FITREP Instruction Block 21: BLANK Block 22: primary graders make billet recommendation for next semester. If no billet is recommended, complete with MIR Blocks 23-24: graders enter comments in upper and lower case format. Do not include the name of the person being graded multiple times. Comments will be in 10pt Times New Roman font. Graders name will be entered in all caps in the following format: MIDN X/C FI. MI. LASTNAME • Ex. MIDN 1/C T. J. WARD

FITREP Instruction • Comments should reflect individual accurately and be positive • Quantifiable is best, but quality is important too • Describe the person, don’t use general comments • Great example of a midshipmen. • Shows potential. • How? • Clippard has stepped up to act as platoon sergeant several times this semester and has taken on the additional responsibilities of this position when needed. He has shown great potential for this billet in the future. He also pushes himself at every PT session and aspires to one day be able to run as fast as Ward.
