Guru GTY (Guru Tetap Yayasan) • Form S06b dari akun PTK yang telah ditandatangani oleh PTK pemohon NUPTK (asli) • Form S07 (Pakta Integritas) dari akun sekolah, yang ditandatangani Kepala Sekolah bagi Guru, ditandatangani oleh Pengawas bagi Kepala Sekolah dan di stempel asli (cap basah), serta ditandatangani oleh PTK pemohon NUPTK diatas materai Rp. 6.000,00 (asli) • 1 lembar fotocopy Kartu Keluarga PTK • 1 lembar Fotocopy SK pengangkatan sebagai Guru Tetap Yayasan selama 4 th • berturut-turut terhitung 1 Januari 2009 yang dilegalisir oleh yayasan • 1 lembar fotocopy SK Penugasan/Pengangkatan PTK di sekolah induk dalam • 5 tahun terakhir oleh Yayasan • 1 lembar fotocopy akta Pendirian Yayasan • 1 lembar fotocopy ijazah terakhir yang dilegalisir • Surat pernyataan belum pernah memiliki NUPTK yang ditandatangani oleh operator NUPTKKab/Kota dan PTK pemohon NUPTK
