Care Coordination Units • Designated by the Illinois Department on Aging • Conduct Comprehensive Care Coordination Assessment • Create plan of care • Goal to maintain independence in a community environment • Assess cognitive skills, environment, health, formal and informal support • Community Care Program (CCP) • 60+ • Risk of nursing home placement • Medicaid eligible

  9. Medicaid Waiver Services • Major component of the Community Care Program (CCP) • Assessment and care coordination (CCU or MCO) • In Home Services • Adult Day Services • Emergency Home Response

Wrap Around Services • Aging Network provides services that wrap around waiver services to keep people in their homes • Home delivered meals • Chore/home repair • Informal caregiver support • Information and Assistance • Respite • Transportation • Health promotion and disease prevention • Many other programs offered through the AAA

  12. AgeOptions Functions • Grant $ to local agencies who provide direct services • Provide oversight and technical assistance to grantees • Plan for future needs and services for older adults • Provide information and assistance • Conduct special projects

  13. AgeOptions • Reached 234,231 people in 2011 • More than the 60+ population in Alaska & Vermont combined • Distribute nearly $16 million to agencies throughout Suburban Cook County • 38 local funded partners with multiple sites in the community
