UCRs A Code: Example Hauling company has a 22 tn minimum 30 tons hauled to soil treatment facility Hauling company bills 44 tons (30 tons plus 14 tons) to achieve 22 ton minimum Pre-approve hauling 30 tons using T075 & T076 Additional 14 tons listed as A-code (A001) Invoice from hauler identifying minimum charge must be submitted with claim Invoiced amount plus up to 18.5% eligible for reimbursement

UCRs Laboratory & Soil Treatment Costs Base costs are diverse Incentive for continued cost savings that the program can share Savings Pass Along Invoiced amount plus one-half the difference between the UCR and the invoiced amount Lab and soil treatment costs will be reimbursed: Invoiced cost + 18.5% ≤ the UCR, or Invoiced cost + ½ (UCR – Invoiced cost) ≤ UCR

  12. UCRs M1376 – UCR $91.56/sample Lab invoiced cost is $60 $60 + 18.5% = $71.10 max allowable reimbursement without Savings Pass Along Savings Pass Along = Cost + ½ (UCR-Cost) $60 + ½ ($91.56 - $60) = $60 + $15.78 = $75.78 Claim $4.68 (6.5%) more than $71.10 Fund saves $15.78 over the UCR
