Connected Care is about doing things differently • Engages the community to define their needs • Provides a detailed understanding of the breadth and depth of needs within communities • Fits with the current and emerging government and commissioner agenda • Leads to improved joint working between health and social care services

Case study Swindon Background • Commissioned by NHS Swindon and Swindon Borough Council • 12 Month project – Jan 2011/ Jan 2012 • Three areas – Central Ward, Penhill and Taw Hill • 20 Community Researchers- 13 have completed training course, 7 currently being trained. Designed a questionnaire and now ready to go out and start doing the research.

Case study Swindon Next steps • 4 months to carry out the research • Questionnaires, Interviews, Focus groups • Set up a front line staff forum to involve front line staff in Connected Care • Produce an interim report on findings • Produce a final report with recommendations for changes/ improvements • Work closely with Swindon Borough Council and NHS Swindon to implement recommendations

Impact across 10 areas • Over 160 community researchers recruited and trained • Hartlepool and Bolton: community run social enterprises • Suffolk: frontline staff developing drop-in for health and social care services; carers setting up support group • Basildon: local charity promoting mental well-being • Clacton: community-led timebank proposal • East Lancashire: building capacity of health services to engage with GRTs
