  5. Medical History Connective tissue disorders Immunocompromised state (HIV/AIDS) Corticosteroids or chemotherapeutic agents isotretinoin(eg , Accutane) and amiodarone (cordarone) pacemakers and inplanted defibrillators pregnant and nursing women

  6. Pertinent ocular History Contact lens Patient Age , Presbyopia and Monovision

  7. EXAMINATION Uncorrected visual Acuity and Manifest and cycloplegic Refraction Acuity Pupillary Examination Ocular Motility , confrontation fields , and Ocular Anatomy Intra Ocular pressure Slit – Lamp Examination Dilated fundus Examination

  8. ANCILLARY TEST corneal topography Pachymetry wave front analysis Calculation of Residual Stromal bed thickness after LASIK

  9. Discussion OF FINDINGS AND INFORMED CONSENT
