  2. DP Transition Year Data Submission Reference Card On your scheduled submission dates you are required to submit an e-mail to the Data Coordinator at the Coordinating Center. See your assigned group and your group’s submission calendar on the SDPI DP Program website: http://aianp.ucdenver.edu/sdpi/dp/data/new/dp_new_data.htm If you are NOT submitting forms: • Prepare the submission e-mail • Name your e-mail subject line like the following:55101.1E_Sub9.1.2009. • Include a note in the body of the e-mail that no forms will be submitted. • Send the submission e-mail to the Coordinating Center on your scheduled date. If you ARE submitting forms: Step 1: • Check completeness of each form you are submitting • Make sure to have the NDPID #, Site #, and Participant ID # on each form. • For Comprehensive (C1 and C2) forms, make sure to indicate which assessment visit is being completed (in the header of the form). • For each form other than the questionnaires (C2, F2, F3, A2, A3), make sure every field on the form is filled in. • Double-check there is no PHI (Protected Health Information) on any of the forms. • Make a copy of each form to be submitted for your records.  Step 2: • Fill out the Tracking Logsheet • Name your file like the following:55101.1 DP Tracking Logsheet 9.1.2009. • Only include Participant IDs/information for those participants for whom you are submitting forms. • Use the left column for any new or original participants with forms to be submitted and the right column for any transition participants with forms to be submitted. • Print one copy of the Tracking Logsheet for your own records and one for the package. Step 3: • Prepare the submission e-mail • Name your e-mail subject line like the following:55101.1 E_Sub9.1.2009. • Attach the Tracking Logsheet to your e-mail • Ask another staff member of your program to double-check all forms that are being submitted to ensure they have been de-identified • If everything is de-identified, ask her/him to type a sentence in the submission e-mail certifying that s/he has double-checked all materials being submitted. • Send the submission e-mail with the certification to the Coordinating Center on your scheduled dates. Step 4: • Prepare the package • Only send a package in the mail if you have completed participant forms to submit! • Double-check that the forms to be submitted match those listed on the printed Tracking Logsheet. • Mail the package to the Data Coordinator at the Coordinating Center. • Sample mailing labels are available on the SDPI Program website: http://aianp.ucdenver.edu/sdpi/common/new/DP_Appendices/MADPG%20DP_CC_DC_%20Mailing%20Labels.docThe labels were developed based on Avery #5163.
