  2. Aug. 02 – Aug. 08 Birmingham August 4, 2014 Contractor was welding at cooling tower and a spark ignited an outside perimeter bait station. The fire watch extinguished small fire immediately, no physical damage. Hot work permit was issued; however, the bait station was not immediately relocated from beneath the welding area.

  3. Aug. 02 – Aug. 08 Kent August 5, 2014 EE received electrical shock from depal control panel. ET found exposed 12V wire with worn casing. Wire replaced and electrical system audited to ensure no other wires were worn.

  4. Aug. 02 – Aug. 08 Fairfield August 6, 2014 Employee attempted to pull the hub off the back of the printer using the wrong forklift. In addition the Lift was not chained off to the forklift. Stopped immediately and got the correct forklift.

  5. Aug. 02 – Aug. 08 Phoenix August 8, 2014 FE Mechanic was removing an ironing ring from bodymaker when it slipped from her hand, bounced on the grating, and struck her ankle. EE was taken to Occ Clinic, diagnosed with bruised ankle and given OTC medication.
