  4. GOALS A1C – every 3 months Blood Pressure – every 3 months Weight (BMI) every 3 months Physician / Clinic visit – every 3 months Foot exam – annually Eye exam – annually

  5. Standards Established: • A1C – less or equal to 7.0 • Blood pressure – 130/80 (normal range) • BMI – 24.90 - Below 18.5 – Underweight - 18.5 – 24.90 = Normal - 25.0 – 29.9 – Overweight - 30.0 – above - Obese STANDARDS ESTABLISHED

Each participating clinical team had agreed to openly share patient data for sample of patients being seen at their practice.
