Little old lady with little old vein grafts: History • 81/F with SVG to LAD, PDA, OM1 and OM2 in 1990 • Hypertension, Hypercholesterolemia, hypothyroid • Preop workup for cholecystectomy showed positive • Nuclear test with reversible ischemia in postlat wall. • Cath shows 70% lesion in SVG to OM1 and 100% in • SVG to OM2. • Post op had one episode of chest pain – repeat nuclear • Study same as before – for cath. Little old lady with little old vein grafts : Take Home Points • Don’t panic, keep calm. • Inflate balloon to stop further bleeding. • Wait out/ reverse anticoagulation. • Prolong balloon inflations. • Covered stents. • Beware of little old ladies with little old vein grafts – they might explode.
