PREPARING for 7th Grade State immunization requirements: Tetanus/TDap Booster Meningitis Vaccine 2nd Varicella Vaccine • If you are unsure about your child’s immunization status, contact your physician’s office. Tell them your child will be entering the 7th grade to ensure compliance for next year’s requirements. • The immunization records can be submitted to the clinicvia fax, e-mail, or in person beginning now! *Please send all immunization records to the MIS/HPMS nurses and Athletic physicals to the coaches! Athletics: District Wide Physicals will be offered May 18th at Highlander Stadium. Cost is $20.00 cash or check payable to HP Sports Medicine.
