  2. What Is Healthy People? • A national agenda that communicates a vision for improving health and achieving health equity. Some Features of Healthy People 2020 • Creates a strategic framework - a “roadmap”for nationwide health promotion and disease prevention. • Provides a set of data-driven objectives to monitor progress and to motivate, guide, and focus action. • Guides national research, planning, and policy efforts.

History of Healthy People • 1979—ASH/SG Julius Richmond establishes first national prevention agenda: Healthy People: Surgeon General’s Report on Health Promotion and Disease Prevention • HP 1990—Promoting Health/Preventing Disease: Objectives for the Nation • HP 2000—Healthy People 2000: National Health Promotion and Disease Prevention Objectives • HP 2010—Healthy People 2010: Objectives for Improving Health • Healthy People 2020– Launched December 2010

Healthy People 2020 Vision: “A society in which all people live long, healthy lives.” • Mission: Healthy People 2020 strives to: • Identify nationwide health improvement priorities. • Increase public awareness of the determinants of health, disease, and disability and the opportunities for progress. • Provide measurable objectives that are applicable at the national, State, and local levels. • Engage multiple sectors to strengthen policies and improve practices that are driven by evidence and knowledge.

Healthiest Wisconsin 2020 • Wisconsin Statute Section 250.07(1)(a), requires the Department to produce a public health agenda for the people of Wisconsin at least every 10 years. • Healthiest Wisconsin 2020 is an ongoing process using science, quality improvement, partnerships, and large-scale community engagement. • More than 1,500 people participated in the development of this plan. www.dhs.wisconsin.gov/hw2020

HSC 2020 Mission & Vision Mission: “To improve the health of the people of Sheboygan County.” • Vision:The vision is accomplished by following the following objectives: • Increasing the community's awareness of public health issues specific to Sheboygan County. • Identifying existing resources through organizations, agencies, citizen groups, and health care providers addressing specific public health issues. • Forming meaningful, action oriented coalitions of existing resources to reduce fragmentation effort in addressing Sheboygan County quality of life issues. • Mobilizing our community's residents in taking responsibility for adopting healthier lifestyles.
