Department of Interior Bureau DASHO(s) have delegated authority to Fire Directors at NIFC for wildland/RX fire serious accident investigations including: Entrapments/Shelter Deployments Fatalities/Multiple Serious Injuries Substantial Loss of Property Incidents with Potential and/or Non-Serious Injury

  7. Forest Service Wildland/RX fire accidents or entrapments are Chief’s Office level investigations which may be delegated to the Region by the DASHO.

  11. Investigation Categories Serious Wildland Fire Accidents One or More Fatalities 3 or More Hospitalized from Incident Property or Equipment Damage > $250,000 DASHO Judges a SAI is Warranted Non-Serious Wildland Fire Accidents Near-Miss Entrapment Fire Shelter Deployment

  12. Unit Responsibilities Complete Wildland Fire Entrapment/Fatality Report (PMS 405-1), Exhibit 5-2 Complete 24 hour report Submit to National Interagency Coordination Center (NICC) within 24 hours.

  13. Team Organization Wildland Fire Serious Accident Investigation Team includes the same positions as other investigations. SAIT will be composed of fire and non fire personnel

  15. Visits to Incident Visits to Incident Command Post Requirements for Personal Protective Equipment (PPE) Lace-up boots with traction soles Long trousers Long sleeve shirt Agency Uniform is Acceptable

  16. Visits to Fire Line Advance notification shall be given to the Incident Commander or delegated representative prior to travel to fireline. IC will assign a liaison to SAIT to ensure coordination takes place. SAI members must maintain communication with fireline supervisor in area they are located.

  17. Visits to Fire Line SAI Teams will need specific PPE and basic training as required by NWCG. Required PPE 8-inch high, lace-type leather boots Aramid shirt & trousers Hard hat with chinstrap Leather gloves Fire shelter (new generation Jan 2010)

  18. Visits to Fire Line Non-Escorted Physical fitness level of “light” Must have adequate communications and radio training Annual Fireline Safety Refresher Training Completed the following training: Introduction to Fire Behavior (S-190) Firefighter training (S-130)

  19. Visits to Fire Line Escorted Visits All non-incident, non-agency, visitors lacking above training and physical requirements must be escorted while on the fire line. Must receive training in proper use of PPE Must be able to walk in mountainous terrain and be in good physical condition with no limiting physical conditions Escorts must be minimally qualified at the Single Resource Boss. Any deviation from this requirement must be approved by the IC.

  20. Helicopter Flights • All passengers need a briefing • Need qualified HECM on board to load/unload at site • Required PPE • Flight helmet • Leather boots • Fire resistant clothing • Leather gloves/Flight gloves

  21. Accident Site Activities Investigators should examine PPE involved in the wildland/prescribed fire accident. Did Firefighter(s) use full PPE? Did Firefighter(s) remove any of PPE or use it improperly? Were Fire Shelters used? What Procedures were used? Analysis of PPE should be inspected for compliance with agency policies.

  22. Accident Site Activities Clothing and equipment subjected to radiant heat or direct flame should be compared with industry examples to show temperature ranges in the deployment, entrapment, or fatality. Assessing PPE – Exhibit 5-3

  23. Accident Site Activities Order an Equipment Specialist from the Missoula Technology and Development Center (MTDC) Request through agency Fire & Aviation Safety Manager Wildland Fire Fatality and Entrapment Report (Exhibit 9-2)

  24. Documentation Dispatch Logs & Recordings Unit Logs Incident Action Plans Agency Land Use Plans Fire Management Plans Prescribed Burn Plans Contingency Plans (if applicable) Risk Assessments/Job Hazard Analysis Documentation of work/rest guidelines
