Case • “Maggie” • 7 Yr, FS Yorkshire Terrier • Lethargy, Inappetance • Panhypoproteinemia (Albumin 1.1, Globulin 1.9) • Hypocalcemia • Hypocholesterolemia
Abdominal US • Mildly thickened SI walls: (duodenum: 5.7 mm, Jejunum: 5.0 mm) • Multiple transverse SI mucosal linear hyperechogenicites • Thickened SI mucosal layer w/ mild loss of layer distinction (subjective) • Moderate anechoic peritoneal fluid (ascites)
Histopathology • Sx biopsies: stomach, duodenum, jejunum, ileum • Duodenum: mod. lymphangiectasia and mod. lymphoplasmacytic enteritis • Jejunum, Ileum: mod. lymphangiectasia and mild lymphoplasmacytic enteritis
Lymphangiectasia • Obstruction of lymphatics • Lymphatic dilation and rupture w/ loss of lymph (proteins, chylomicrons, lymphocytes) into int. lumen • Panhypoproteinemia – ascites, S.Q. edema, pleural fluid • Hypocholesterolemia • Lymphopenia
Lymphangiectasia • Soft-coated Wheaten terriers, Basenjis, Yorkshire terriers, Norwegian Lundenhunds • Diarrhea, weight loss, vomiting, ascites, S.Q. edema, dyspnea (pleural fluid) • 1°/Congenital • Focal – S.I. only • Diffuse – Chylothorax, lymphedema, chyloabdomen • 2° • RCHF, Constrictive pericarditis, S.I. neoplasia (LSA), IBD
Lymphangiectasia • Dx: endoscopy/laparotomy w/ histopath • Tx: oncotic support, low fat diet, Tx of 1° dz

VRUS 2007 • Retrospective study, 23 dogs w/ SI hyperechoic mucosal striations and bx • Histopathologic lacteal dilation was present in 96% • Additional US findings: mild jej. wall thickening (96%), mild duod. wall thickening (78%), mucosal speckles (70%), and abd. effusion (87%) • Striations were diffuse (70%) or multifocal (30%) • No loss of wall layering, except in one dog with a severe mural lipogranuloma • Inflammatory infiltrates were mild (61%) or moderate (30%) • Chem. consistent with protein-losing enteropathy in 78%
VRUS 2007 • Only 1 dog w/ striations but no evidence of lacteal dilation • Mild, multifocal interstitial villus edema with mild neutrophilic and eosinophilic villus inflammation
Disseminated villus (interstitial and lymphatic) histiocytic sarcoma with marked lymphangiectasia (neoplastic cells within the dilated lymphatics)
