State Performance Plan • Not later than 1 year after the date of enactment of IDEA 2004, each State shall have in place a performance plan. • Each State shall establish measurable and rigorous targets for the indicators established under the priority.
Percent of youth aged 16 and above with an IEP that includes coordinated, measurable, annual IEP goals and transition services that will reasonably enable the student to meet the post-secondary goals. Indicator 13: Effective Transition
