Transformation & Delivery Darlington Clinical Commissioning Group – Plan on a Page 2014/15 to 2015/16 Vision Local Outcomes Strategic Aims Priorities Commissioning Intentions Improve the health status of the people of Darlington Adult Social Care Outcome Mental Health & Well Being Darlington Operational Model • Support the creation of a ‘Healthy Darlington’ where people eat well, move more and live longer and ensures that every contact counts • Tackle health inequalities to better support socially disadvantaged communities and vulnerable groups • Improve the both the mental and physical illness of carers U75 Cancer mortality To address the needs of the changing age profile of the population of Darlington • Take forward the ‘Care Home’ Project • Progress the model of care for community stroke services • Review of County Durham and Darlington Home Equipment Loans Service (HELS) to ensure that the business model is fit for purpose, supporting the shift of care that can be managed outside of hospital • Take forward the Palliative Care Strategy to ensure that people have choice dignity and respect at the end of their lives. Elderly Frail Primary Care Collaborative NHS Constitution To secure the right services in the right place for the people of Darlington • Invest in improved care for elderly people with complex conditions and others with long term health problems • Establish a ‘Clinical Challenge Fund for a Long Term Condition ’ [COPD, Cardiology, Diabetes or Dementia] that brings together primary and secondary care clinicians to deliver better care for people • Secure the co-location of urgent care with emergency services as the first step towards an improved model for urgent care and intermediate care services • Deliver a shift in the delivery of care and support to people with a learning disability into the community • To implement recommendation from the ‘Securing Quality in Health Services’ and development of seven day services • To mainstream the successful schemes and other pilots that demonstrable improve care, services and the management of care e.g. Front of House; Parent education; Professional education; paediatric led rapid access clinics • Improve childrens’ emotional and mental health through service improvement and better transition to adulthood. Securing Quality in Health Services Long Term Conditions Emergency admissions for children with lower respiratory tract infections Working together to improve the health and well-being of the people of Darlington To manage our resources effectively Urgent Care Unplanned hospitalisation for asthma, diabetes and epilepsy in under 19s Integrated Transformation Teams To invest in primary care and services • Invest in primary care infrastructure and wrap care and services around practices • Improve access to primary care services seven days a week Securing Quality To secure meaningful engagement with people HCAI • Provide structured engagement and feedback on care and services • Invest in local engagement with communities and businesses Better Care Version 1 23/07/2013
