  2. Integrated or Collaborative Approach Occurs when individuals from two or more SPECIALITIES learn about… learn from… and … learn with each other to enable effective collaboration in patient management and improve health outcomes.

  3. Integrated Approach : Why is it needed ? • Traditionally the Individual care giver is solely responsible for a patient safety • But an integrated approach improves patient outcomes • Each member brings in their area of expertise and skills to complement each others thus enhancing the quality level of patient care • Delivery of care becomes a shared responsibility

  4. Integrated Approach • Interdisciplinary:Team members from different disciplines working collaboratively with common purpose, to set goals, make decisions and share responsibilities • Multidisciplinary: Team members working independently to create discipline-specific care plans implemented simultaneously • Combination: Can be called Intergrated Approach

Benefits of an Integrated Approach to Care • Increased patient perception • thatthey are cared by a skilled • team • Greater likelihood of the delivery • of care in accordance with • national standards and clinical • practice guidelines • Increased patient satisfaction • Better utilization of skills and • resources • Reduction in hospital stays, • unwarranted interventions, • investigations and therapies

Barriers to Integrated Approach or Strategy • Resistance to change by the clinicians • A lack of time on the part of clinicians to attend multidisciplinary meetings • Issues of patient ownership • The unknown costs involved • The difficulty of establishing such approaches in remote areas because of a lack of team members.

  14. SUMMARY • Teams do not replace the physician-patient relationship, but rather enhance it—creating a more comprehensive, efficient, and tailored health care experience. • A team-based approach can serve you better by changing the culture from “my patient” to “our patient.”
