This initiative aims to ensure all enrolled Ontario teams submit baseline data by September 30, 2006. We strive to accomplish measurable improvements through a consistent submission of data, allowing us to track our monthly progress. To facilitate this, we will establish contacts with designated teams and serve as a resource to help them effectively collect and submit their data. By coaching these teams through the change process, we encourage continuous learning and impactful quality improvement within our Local Health Integration Networks (LHINs).
