  2. We’re in this Together! 45 Day New Student Screening Presentation to TUSD Leadership Thursday, September 2, 2005

  3. Sign-In, Please! • Please make sure you sign in. • ADE requires that we submit the sign-in sheets to document that all TUSD staff have been trained. • Psychologists are conducting site trainings in collaboration with Principals.

  4. Who? • Think of it as a General Ed. functionthat the Exceptional Education Department is accountable for compliance.

  5. Who? • ALL students NEW to TUSD since May 2, 2005*,MUST have this form in the cumulative folder within 45 days of enrollment. • This includes all new TUSD Pre-K, Kinders, and 1st through 12th graders, even those already in Special Education. • Every student who walks through TUSD doors for the 1st time! *We are not required to go back for students who enrolled prior to May 2, 2005.

  6. ADE Monitoring (R7-2-410-D) • April, 2005 – found out of compliance at most schools visited • End of May, 2005 – compliance had improved but not all schools visited were aware • Fall, 2005 – ADE will be back!

  9. Key Personnel The Office Manager/Attendance Clerk* is the KEY to ensuring COMPLIANCE!(*or principal designee)

  10. Basic Instructions – Section A • Office Manager/Attendance Clerk/Designee runs the Mojave Report Generator every Monday and gives the appropriate form to the assigned teacher • Forms are no longer sent to your site • “Run Dates” listed on “45 Days on a Page”

Basic Instructions – Section A • Section A will be generated automatically by Mojave except for “Screening Date” which will completed by the teacher. • Mojave will automatically generate the date that the form is to be filed in the cum folder. Screening Date filled in by teacher on the day s/he completes the form.

  12. Basic Instructions – Section B • Homeroom or Main Teacher • Fills in the “Screening Date” on Section A • Completes Section B by marking every item • This is a screening, not an assessment. If the teacher has a concern for an item, s/he should mark it. A more thorough assessment will occur during follow-up. • Writes the date on the form when returning it to the Office Manager/Attendance Clerk/Designee

Basic Instructions – Section B • Office Manager/Attendance Clerk/Designee • Checks the form for completeness • If no concerns, either • gets the Principal’s signature and files the report in the cumulative folder, or • can sign for the principal and file the report in the cumulative folder. • If even one item is marked “yes”, gives the form to the Principal for follow-up. • Maintains his/her own system for keeping track

Basic Instructions – Administrative Action • Administrative Action • If concerns are noted, the Principal • Marks the form “Problem noted……” • Notifies the parent of the concern within 10 school days upon receipt of the form from the teacher • Marks the appropriate actions to be taken • Dates the action as of the date the appropriate staff member was alerted • Signs and dates the form • Files the form in the cumulative folder

Basic Instructions – Administrative Action • Notification to Parent can occur in one of the following ways: • Documented phone call • Face-to-face meeting • Letter • 2 templates created by your colleagues are available on the web site • Thank you to Maggie Shafer, Art DeFilippo, and Dan Weisz for sharing!

  17. Compliance All Sectionsmust be completed and the form filed in cumulative folder within 45 days of enrollment.

FAQs (Frequently Asked Questions) • FAQs are now available on the web. • Example: • Question:If the student withdraws before the 45th day, do you have to complete the form and file it in the cum folder? • Answer:You have 45 days to complete student screenings whenever the child remains in the district for 45 days. If the student withdraws before the 45th day, you do not need to complete the screening.
