  1. 2013-2014 Annual Action Plan(Fourth Year of the Five-Year Consolidated Plan) The City expects to receive approximately $2.5 million in entitlement funds in Fiscal Year 2013-2014 Community Development Block Grant (CDBG) - $1.7 mil Emergency Solutions Grant (ESG) – $187,000 HOME Investment Partnership Grant (HOME) - $600,000

  2. Plan Requirements • To receive entitlement funds from the U.S. Department of Housing and Urban Development (HUD), the City of Modesto is required to develop a plan that describes activities for these funds in the upcoming fiscal year. • Activities must be: • consistent with HUD guidelines, and • developed in consultation with the community • Public meetings are being held in April 2013

Address Homelessness • Provide financial assistance to agencies responding to the immediate housing needs for the homeless or those at immediate risk of homelessness. • Essential services to homeless persons • Short- and medium-term rental assistance • Emergency shelter and transitional housing • Assist persons transitioning to independent living to regain stability in permanent housing

Community Development • Neighborhood Clean-Up Projects • Code Enforcement • Repair Curbs and Ramps • Public Facility Improvements • Construction to provide accessible restrooms at local parks • Install ADA playground equipment at local parks • ADA survey of City owned facilities • Neighborhood Revitalization Strategy
