    3. In Reach Team Development. Evaluation showed high number of people with Dementia on general acute care wards. Likelihood of Physical and Mental Health Needs not holistically managed. In patient service for mental health 37 admissions in 10 month period. Need to work smarter and differently

    4. Challenges Month time-scale from Ward closure to new service. Understanding staff requirements some skills and knowledge transferable. Engagement with key partners. Building confidence with patients, carers and the local community. Understanding the needs of the Acute hospital site

    5. Composition of Team. 1 Band 7 (Team co-ordinator) 3 Band 6 (RMN) 5 Band 3 (HSW) supported by:- * 2 Consultant Psychiatrist * Mental Health Occupational Therapists * Mental Health Social Workers. * Alzheimer’ s Society * Links with other aspects of CMHT.

    6. Key Objectives. Holistic assessment Single treatment plan. Two way education Team decision making Timely discharge Follow on support where required.

    7. Key Areas of Integration Attendance/input at multi-disciplinary meetings. Attendance and involvement in discharge meetings Integrated service meetings across all stakeholders. Daily input to wards.

    8. Feedback from Nov 05-Dec 08 Referrals. 741 Questionnaire to general hospital. Questionnaire to Service users. Questionnaire to Carers. General feedback by team.

    9. Total 741 Returned home 390 53% New referrals passed on to CMHT 107 14% (4 per month) Referred back to GP 449 61% (No further action was required) Nursing home 187 25% (Placed or returned) Deceased 114 15% Requiring Mental Health Admission 9 12% Inappropriate Referral 31 4% Out of Area 63 9% (Either discharged or referred on as appropriate)

    10. Feedback from satisfaction surveys ULHT staff. 94% Easy access to In-reach 94% Happy with response speed 100% Professional approach 92% Enhanced care, mental health on wards 100% Beneficial to their service

    11. Service users and carers 89% Treated with dignity and respect 78% Listened to 44% Service was excellent 56% Service good 67% carers, listened to 92% carer, treated with dignity and respect
