  2. CASO CLÍNICO • Kinesiterapia post-disectomía lumbar Paciente S. V. de 45 años, jugador de rugby y de profesión ingeniero civil. Se presenta a tratamiento kinésico con dos meses post cirugía, con sensación dolorosa a la flexión y rotación a la altura de L3-L4-L5 y EVA 4/10

  3. BIOMECÁNICA FUNCIONAL • HNP: protrusión del núcleo pulposo de un disco intervertebral a través de una fisura en el anillo fibroso externo (1) • Disco intervertebral: aneural y avascular. (2) • Articulación Sínfisis: compresión, distracción y torsión (2) • Exterior anillo: reparación tejido (2)

  4. BIOMECÁNICA FUNCIONAL

EPIDEMIOLOGÍA • Vinculada al lumbago, causa más frecuente de jubilación en < 45 años, impacto económico y convalecencia crónica (1) • Cirugía: compresión nerviosa, síntomas persistentes, sin respuesta tto conservador (3) • Prevalencia: HNP sintomática 1-3% Europa. 30-50 años (25-55a 95% L4-L5 y L5-S1) (>60a L3-L4 y L2-L3) (4)

  6. ETIOLOGÍA (5) • Degeneración o envejecimiento articular •  Microtraumatismos • Movimientos de rotación continuados en el tiempo • Atrofia musculatura paravertebral dorso lumbar

  9. PRUEBAS FUNCIONALES (7) • Test de inclinación con rotación • Test de reposición (flexión) y extensión • Test million dollar

  10. LIBERACIÓN MIOFACIAL • Elongación Cuadrado lumbar • Liberación Piramidal, glúteos • Elongación IQT

  11. ARTROKINEMÁTICA • Estudios: basta un 4% de microinsuficiencia para producir disfunción con inestabilidad articular • patrones musculares de Janda: síndrome cruzado distal: unos se inhiben: Multifidos, activan: Erectores espinales, debilitan: Glúteos y abdominales

  12. TRATAMIENTO KINÉSICO • Flexibilización y elongación EEII y tronco • Glúteo > • Psoas mayor • Piramidal • IQT • Erectores espinales • Cuadrado lumbar • Banda iliotibial • Estabilización con esfingomanómetroMultífidos, Transverso abd, Diafragma, Oblicuos • CSM • Masoterapia

  13. TRATAMIENTO KINÉSICO • Reeducación musculatura estabilizadora lumbopélvica (esfingomanómetro, balones, perturbaciones) • Multífidos • Transverso del abd • Oblicuo • Musculatura piso pélvico • Hollowing • Core estabilization

TRATAMIENTO KINÉSICO • Progresión • Aumentar rangos de flexibilización y elongación • Control específico estabilización lumbopélvica en balón suizo • Reeducación resistencia musculatura debilitada • Bicicleta estática • Marcha de bajo impacto • CSM respuestas rápidas, aumentando estímulos • Finalmente actividades de reintegro a la actividad laboral y/o deportivas
