  1. LOCKOUT TAGOUT (LOTO) Awareness Training Program Updated 6/29/04 by SN 1

  2. Objectives: • Be able to • Identify sources of energy • Recognize LOTO signs • Understand policies and your responsibilities 2

  3. Identifying Sources of Energy • There are different sources of energy available on campus: • Mechanical/Electrical • i.e. elevator • Compressed Gas • Thermal • i.e. Steam • If any maintenance work is required, DO NOT ATTEMPT to fix. Contact the Facilities Department at x 4588. • Otherwise, you can submit a work order using the College web page. 3

  4. Definitions • Authorized Employees are the only ones certified to lock and tagout equipment or machinery. Authorized Employees are designated and receive extensive training. • NYMC Employees who are not designated and trained are not authorized employees. • Affected Employees are the people who usually operate the machines or equipment for which lockout or tagging out is sometimes necessary • i.e: person who goes to use an elevator. 4

  5. Policies and Procedures • LOTO is used to ensure that work can be performed safely on machinery/equipment without risk of sustaining injury due to re-energization. • When LOTO is used, someone’s safety or life is on the line. • The person who applies the lock/tag is the ONLY person who can remove it. • NEVER remove a lock or a tag, even if it has been there for several weeks! 5

  6. Recognize LOTO Signs • A lock or tag is used to indicate that the equipment or energy source should not be turned on or activated. • If you see something similar to this picture…DO NOT TOUCH!! 6

  7. Why should we adhere to LOTO? • OSHA predicts that compliance with the lockout standard would prevent: • 140 fatal accidents a year • 50,000 LOTO-related injuries 7

  8. Responsibilities Follow NYMC policies and procedures at all times Recognize and report any unsafe conditions It’s YOUR safety, take ownership! 8
