Maintenance of AF Form 623 • Training Records • Maintain AF Form 623 on all 1C1X1, 13MX and GS-2152 personnel who are position certified, facility rated, or in training. • Maintain the following items in each training record in the following tabular format: • Tab A: All current preprinted and supplemental AF Form 1098s • Tab B: Entries made on AF Form 623a. Enter in the following order:

AF Form 623 Form Disposition • Training evaluations • Position certifications • All other entries (i.e. AETC Form 156, Student Record of Training) • Tab C: Applicable CFETP • Tab D: AFJQSs and Qualification Training Package Documentation • Tab E: MAJCOM and Unit AF Form 797s • Tab F: AF Form 3622 • Tab G: Expired documents for retention (i.e. previous year’s AF Form 1098’s etc)

AF Form 623a Documentation • Name, inclusive dates of evaluation, position start date and total calendar days allowed for position • Specific tasks covered during the evaluation period • Describe STS/797 task trained during this period • Spell it out, STS 7.73. Wake Turbulence Cautionary Advisory • Status of trainee development • Assess the trainee’s progress, or lack of progress toward the block objectives • Include deficient areas within this block of training

AF Form 623a Documentation • Results of previous corrective actions • Be specific • Explain what method was used to correct the deficient area and the results of the training • If no training was accomplished to correct the deficient areas, say so. However, follow up on the next training evaluation to show that corrective actions were completed. • Time spent in each category of training (live, simulator, non-positional) • Other comments • Trainee comment 623a

Responsibilities • AOF/CC reviews and signs each AF Form 623a • CCTLR documents on AF Form 623a corrective actions to be taken on trainees who are not progressing satisfactorily • Document on AF Form 623a when a controller is no longer in “EDIT” status • CATCT reviews and signs all 623a • WS reviews/signs and recommends to the CCTLR to enter a trainee into EDIT • Trainer writes/recommends to the WS to enter a trainee into EDIT

  14. Additional Training Times • CCTLRs may approve additional training times not to exceed 50% of the original time allotted for the position (calendar days) • MAJCOM may approve an additional 50% not to exceed 100% of the original time allotted for the position

  15. Front Load Training (FLT)

Purpose/Objective • Purpose • To introduce a defined amount of knowledge and or performance at the beginning of a controllers training program • Objective • To send a trainee to live traffic with a good understanding of phraseology, separation, and control practices prior to working live traffic • FLT is required for all apprentice controllers

  17. FLT Structure • FLT is structured into four blocks of training • Block I covers local area information • Block II covers facility equipment • Block III covers general ATC practices • Block IV covers practical applicationAFI 13-203 FLT • Kadena FLT (hyperlink)

  19. ATC Withdrawals

  20. Purpose • Failure to earn or to comply with the requirements to hold an FAA ATCS certificate will initiate withdrawal action • Withdrawal Categories • AFSC withdrawals for failure to obtain or maintain a rating • ATCS certificate withdrawals for failure to maintain mandatory qualification standards other than medical • ATCS certificate withdrawals for medical disqualification
