Benchmarking Florida Practices: Key Insights from the CMHI Assessment

This analysis evaluates the performance of Florida practices using the Chronic Care Management Index (CMHI). It highlights key strengths and weaknesses across various domains and themes, revealing the highest and lowest scoring areas. Notably, Chronic Condition Management emerged as the top domain, while Quality Improvement was the weakest. Critical themes, such as Family Support and Cooperative Management, also stood out. Our findings emphasize the importance of teamwork and continual improvement in delivering higher-quality care.
