  2. HISTORIA CLÍNICA • Mujer 86 años • AP: HTA; insuficiencia cardiaca; 2 cateterismos; 3 stent; insuficiencia renal; ooforectomizada; operada de glaucoma y cataratas • Enfermedad actual: tumoración mamaria (CSE) de 6cm de 1 año de evolución que crece progresivamente

  4. DIAGNÓSTICO METÁSTASIS DE MELANOMA

  5. TUMORES METASTÁSICOS EN LA MAMA • Mtx a mama desde tumores extramamarios raras • Infancia: RMS embrionario • Adultos: leucemia/linfoma/MM/carcinoma • Incidencia: 2% • 40% primera manifestación

  6. Nódulos solitarios, CSE, superficiales. Gg axilares + (25-80%). Unilaterales • Pruebas de imagen: circunscritas, no calcificadas. Av indistinguibles de 1º mama • Localización 1º: PTS Y MMSS (vía linfática) • Impt buena correlación cx-rx y sospecha: aspecto inusual para 1º mama/ausencia componente in situ/características típicas de o: primario: pigmento/ inclusiones intranucleares) • IHQ: S-100 (S:> 95%). CA mama 60% HMB-45 y Melan A: S: 70% Citoqueratinas – (av expresión aberrante de CAM5.5 y EMA)

  7. Tto y evolución diferente • Muerte 1º año desde dco • Pronóstico dep de sitio 1º y tipo histológico
