  2. Rationale • Ukraine is planning to implement the health care reform: situational analysis • Ukraine has just implemented the administrative reform, which provides more authority to municipalities

  3. Rationale • Estimate the structure and scope of HIV/AIDS expenditures on the local level (oblast, district, city) and compare it to National expenditures structure • Strengthening capacity of NGOs and NGO networks through development of budget monitoring methodology

  4. Background • Legal base: • Constitution • Budget code • National programs • Reporting: • Budget classification (problematic) • Local budget vs budget of health care facilities

  5. Background • NHA (2003-2004) and NASA (2003-2005) experience for the National level study • HIV/AIDS related statistics (second generation epidemiological surveillance, routine surveillance, MARPs size estimations, data on MARPs coverage)

  6. Goal • Evaluate the role of local budgets in the area of HIV/AIDS with active involvement of local NGOs • Strengthen the role of civil society in the local level budgetary process

  7. Objectives • Adopt NASA methodology specifically to local level budgets • Raise public sector participation in HIV/AIDS expenditures analysis • Use the results of study in advocacy process

  8. Objectives • Assess the structure and scope of sampled local HIV/AIDS-related expenditures • Conduct comparison analysis of local and National expenditures structure and scope • Define gaps in HIV/AIDS related funding • Evaluate the efficiency of funds utilization

  9. Scope • Year: 2005 • HIV/AIDS related expenditures: • MoH, MoFY, Penitentiary Dept, MoSA, etc. • External funding • Private expenditures • HIV/AIDS related outputs: • Drugs • Test kits • Etc • Expenditures will be evaluated on the local level (oblast, district, city) – at least 1 site of each level • Beneficiaries analysis

Methodology • NASA framework • Quantitative / Qualitative study • Data collection will include: • analysis of budget reports • questionnaires for donor organizations • survey of out-of-pocket payments • Key respondents: • Local administration • Donor organizations • NGOs • Regional AIDS Coordinating Councils

  11. Limitations • Out-of-pocket expenditures study • Single functional code for HIV/AIDS, TB and Oncology financial reports • Private and corporate donations • Potential risk of double-counting on the NGOs level • Health care facilities budgets: special survey (costing study)

  12. Technical assistance and support needs • External consultant to conduct: • Needs assessment • Cost-effectiveness analysis

  13. Advocacy Strategy • Objectives: • Availability and transparency of financial and funds utilization data • Participation of civil society in budget development and analysis process • Focus on cost-effective approach in budget development process

  14. Advocacy Strategy • Primary audience: AIDS Coordinating Councils, NGOs, Local Administration, Mass-media • Approach: Stakeholders meetings, Regular ACC Meetings, Press-conferences, Publications • Alliances: ACCs, NGOs • Messenger: ACC
