Person Centered Medical Home Recruitment Status • 193 Practices contacted since 1/1/12: • 51 practices enrolled in the program • 82 prospective practices in open status • 28 practices on a Watch-List • 30 practices on a Closed-List • EHR Status of 82 practices in open status: • 51 practices (62%) have live EHRs, purchased or implementing an EHR • 8 practices (10%) are searching for an EHR • 4 practices (5%) have no EHR • 19 practices (23%) the EHR status is unknown at this time

PCMH Recruitment Status - continued • 3 new PCMH and Glide Path practices since the last committee update (February 13, 2013): • TLC Pediatrics, LLC – 2 site, 2 providers • Torrington-Winsted Pediatric Associates, PC – Glide Path, 3 sites, 6 providers (Litchfield County) • Tritown Family Practice, LLC – PCMH Level 2, 1 site, 1 provider (New London County) • 7 practices advised PCMH applications are being worked on for possible submission this month or next: • Barochia Internal Medicine (Hartford County) • Family Practice and Internal Medicine (New Haven County) • Fairfield County Integrative Family Medicine, LLC (Fairfield County) • Grove Hill (Hartford County) • New Britain Pediatrics (Hartford County) • West Hartford Medical Center (Hartford County) • West Haven Medical Group (New Haven County)
