1 – Storefront Group • Mission: • At Storefront we have a long history of serving our communities and improving the lives of the individuals and families within them. We are involved in working with schools and communities and we offer a wide range of clinical services to children, teens, adults, and families. • Locations: 6425 Nicollet Ave S Richfield, MN 55423; and 120 E Hwy 13 Burnsville, MN 55337
Storefront Group • http://www.storefront.org • Our Roles and Responsibilities • educate Somali community members on importance of immunization, myths, importance of record-keeping, requirements for school enrollment. • Time commitment: 1x month; 3hours/day. • Other events: • Diabetes and medication education • mental illnesses and depression • Obesity • Mediation adherence • Youthful promiscuity and STDs and/or HIV • Scheduled dates: 02/27, 04/03, 05/01, 06/05 from 5:30-8:30pm
2 – Minnesota Asian/American Health Coalition (MAAHC) • Mission/Purpose • To advocate for the health and well-being of the Asian/American and Pacific Islander community of Minnesota. • http://www.maahc.org/index.html • About MAAHC • Location: 3702 E Lake ST, Ste 150, Mpls MN 55406
MAAHC • Our Roles/Responsibilities: • Assist in planning and development (P/D) of health literacy assessments (2-3 students) • Outreach to community members • Questionnaires/assessments will focus on how much community members ( Cambodian, Hmong, other refugees) know about their drugs. • Time commitment: • P/D: 2X/month - 1.5hrs/ day; • 02/11 from 8-9:30am; 02/11 from 10-11:30am; 02/11 from 2:30-4pm; 02/12 from 9-10:30am • Outreach: 2X/month - 2hrs/day; details will follow post P/D.
3 – Southeast Asian Ministry • Mission: • Provide general and personal health education to Hmong, Cambodian, and Karen communities, as well as serving as a liaison for community health resources. • Establishing trusting relationships with their clients in an effort to address healthcare issues. • Most community members are victims of torture and abuse /PTSD. • http://www.seam-stpaul.org/
SeAM • Location: 105 W University Ave, St Paul, MN 55103 (opposite Capitol Bldg) • Our roles/responsibilities: • Educating Cambodian community members about their medications. • Prepare and deliver power point presentations on: • the U.S. insurance system and how it works (mostly Medicare and Medicaid) such as reimbursement issues, prior authorizations, drug formularies, donut hole etc • Medication education and side effect management: Psychotropic, HTN, hypercholesteremia, and diabetes medications. • Time commitment: 1X /month or bimonthly – 2-3hours/day • Presentations will either be on Mondays and Thursdays from10-12pm; or Saturdays anytime between 10am and 2pm and last 2 hours max. • Patient home visits will occur at a later time TBD
