  2. PROVISIONAL ACCREDITATION HEP required to submit: MQA – 01 • PART A : General information on the HEP • PART B : Description of programme eg : name, level, duration etc • PART C: Programme Standards pertaining to the 9 areas of evaluation and the standards in each of them.

  3. FULL ACCREDITATION HEP required to submit: MQA-02 Updated version of PART A, B & C as well as PART D – Self Review Report Submissions may need to include relevant attachments, appendices and supporting documents

  4. PART A: General Information on the Higher Education Provider • PART B : Programme Description • PART C : Program Standards • PART D : Self-Review Report

  5. PART D PROGRAMME SELF-REVIEW REPORT HEP prepares a Programme Self-Review Report (PSSR): • Narrative findings of internal quality audit conducted by the department • Guided by Section 4.1 of the Code of Practice

  6. In summary, the Self-Review Report should include the following: • Strengths of the programme in meeting its goals • Areas of concern that need to be addressed • Strategies for maintaining and enhancing the strengths of the programme; • Steps that have been taken to address the problem areas and • Conclusions and recommendations for improvement
