  2. Overview • Collect data on student visitation to Dean’s Office • Google Forms are easy to access thru any internet connected device • Automatically aggregate and disaggregate data • Analyze data to maximize intervention strategies

  3. Google Forms • Dean’s Office form utilizes Google Forms • Easy to step up and FREE! • Form is live on internet • Data filled into forms are collected in spreadsheet on Google Docs

  4. Download the Template • Access Google Docs Home • Enter “TICAL” in the search field and click “Search Templates” button • Click on “Use this Template”

  5. Looking at the Live Form • Birthday/ Student ID optional • SLC/Org. Unit also optional but can be used to spot trends • Password field used to validate form submission , optional as well.

  6. Strategies for using the Form • Option 1- Place link to form in secure location for teachers to fill out, then student is sent down. • Option 2- Teacher sends student down with paper form, which is then inputted in the Google Form by Dean or office clerk. • Both options have advantages/disadvantages

  7. Sharing • Share spreadsheet using person’s Google Account email address • Teachers only get link to live Form

  8. Data Sheet • All submissions go to this page • Edits to data can only be made here • Submissions are time stamped • Column M provides a follow-up marker/ reminder • Column N provides a location for Admin/Dean comments

  9. Stats-Today • Real time disaggregation of submission made today • You can view all data elements including grade, period, behavior, intervention strategy, etc. • Fully automated

  10. Stats-Overall • Time and SLC based disaggregation allows for spotting of trends in short or long intervals • Simply change the “Start Date”, “End Date”, and SLC. • SLC is optional, leave blank for all entries

  11. Sort-Teacher • Sorts sheet by teacher name, thereby putting all entries from a particular teacher together.

  12. Student Search • Search student entries by multiple criteria (Start Date, End Date, Student Last Name, Grade, DOB / ID) • All fields are optional except dates

  13. Teacher Search • Input start and end date • Input teacher name • Disaggregates the data to view students sent by a particular teacher

  14. FollowUp Needed • Insert an open and close date • This sheet will pull entries marked with a “yes” in Column M from the Data sheet • Note: no editing can be performed in this sheet. All editing has to be completed on the Data sheet.

  15. Customizing the Form • In Forms Editor, change the title of the live form • Insert teacher names • Change grades or periods • Change names of SLC/ Org. Units • Change color scheme

  16. Tips and Tricks • A green cell indicates you change the value in the cell • Very easy to use on iPad, or other mobile devices

  17. Clear Sample Data • Select all cells with sample data • Right Click, “Clear Rows”

  18. Conclusion • Form is online • Data is disaggregated dynamically • Helps with spotting trends to maximize Intervention strategies
