  3. Performance Reports are in Microsoft Word & Excel – only complete one or the other not both Tab through the report automatically. In Excel any cells with “0” have formulas to automatically calculate Report will be available by December 20 on the VSS website for download: http://victimservicegrant.iowa.gov/ Click on “Performance Reports” and scroll down to the Performance Report section. Click on appropriate report. Format

Written Instructions/Guidelines • There are written instructions that can be downloaded to be used as a guide in assisting your completion of the VSS Bi-Annual Performance Reports. • Page 1 & 2 of the Bi-Annual Performance Report Instructions/Guidelines provide you with basic instructions on tabbing through the Performance Reports as well as unprotecting the document when necessary. • Instructions are available from the VSS website: http://victimservicegrant.iowa.gov

  6. Technical Assistance Section • Examples could include: • Fiscal technical assistance • Questions on Performance Reports • Help collaborating with other community agencies • Filling out required forms

Narrative Section – Question #1 Summarize innovative projects, program successes, and/or positive changes. • This is an opportunity to sell your program or talk about what has worked. • This provides you with an opportunity to share some of the successes of the program. • Please write a minimum of a paragraph for each question.

  8. Narrative—Question #2 for Victim Witness Coordinators at are VA Funded • Provide one narrative example of how the program services assisted crime victims. • Discuss the VA-services you provided. • Use Service Statistics Section (Section C) as a guide. • Do NOT use victims name or other personally identifying information • Examples can be found in the Instructions/Guidelines

Narrative—Question #3 (optional) • Describe assistance your program has provided to clients that your would like to share with VSS staff • This is where you can describe assistance you have provided that may not be reflected elsewhere in the report • If you have an example you would like to share, but doesn’t fit under service statistics or other parts of the narrative, this would be the place to share it

  10. Media Section • Tells us the number of times your agency/program has had contact with the media in regards to crime victim information. • Count contact with media if you are putting an advertisement in the paper regarding your services. But only count this once not every time the advertisement runs in the paper. • Count it if you have contact with the media to discuss SA, DV, Crime Victims, Statistics. • Count if you have contact with the media regarding doing a television or radio interview or newspaper article.

  11. Presentations • Report the number of presentations given and the number in the audience • Presentations are considered to be when an individual provides general information about the services offered by your program. • For example: presentations conducted to the Kiwanis Club, Rotary Club, Women’s League, VFW, etc.

  12. Training • Report the number of professionals trained. • Report the number of training sessions conducted. • Training is providing more in-depth information with the specific objective and intent for the receiving party to learn and potentially improve their work with victims of crime. • Provided by your staff to other professionals in other agencies. For example: • Training to law enforcement officer, • Training to advocates, etc.

  13. A. New Victims Served by Type of Victimization • Count the victim only once during the contract period: • July 1, 2011 – June 30, 2012 • Do not report by “charge” but by relationship between victim and offender

  14. B. Demographics • Total from the Race/Ethnic Group section should add up to TOTAL VICTIMS from Section A of the previous page. • Total from Gender section should add up to TOTAL VICTIMS from Section A of the previous page • Total from Age section should add up to TOTAL VICTIMS from Section A of the previous page. • Total from Other Demographics can add up to more than the TOTAL VICTIMS from Section A of the previous page.

  15. Service Statistics • How many victims received each type of service once during the contract period (July 1, 2011 – June 30, 2012) • Each item has a description/definition next to it for your convenience. • You count a victim only once during the contract period

  16. Transportation Trips • Provide the total number of transportation trips For example: • You take victim a victim from their home to court and then court to their home. This would be counted as 2 trips. • You take a victim from court to the law enforcement agency back to court and then home. This would be counted as 3 trips.

  17. Submitting the Reports • Submit the reports to your Primary Victim Services Support (VSS) Program Contact. • Submit report via e-mail • If you need an extension please contact your Primary Contact as in some cases we can provide extensions & in others we cannot. • VSS contact will send you confirmation of receipt of your Performance Report • VSS Staff will review your Performance Report within 60 days of receipt and inform you if there are any questions or needed clarifications.

  18. Submitting Reports Continued • Do NOT FAX in reports unless you are requested to by a member of the Victim Services Support (VSS) staff. • Exception: If your agency has a database system that tracks the numbers in the order of your performance report, you can submit this via fax. • Remember to address the fax to your primary VSS contact and it has the program information and the VWC information on the document or fax cover sheet. • Narrative: We still prefer that you email us the first section or narrative section of the report to your primary VSS contact.
